Invisalign Success Depends on Wearing Them Consistently｜9-Month Real Treatment Case of Crowding & Underbite

Today, I would like to introduce a case involving the correction of crowded teeth.

With conventional wire-and-bracket orthodontic treatment, such as Clippy-C, patients only need to be careful with hard foods so that the appliance does not come off.

In short, it is an orthodontic treatment that works well when the aligners are worn consistently.

Today, I would like to introduce the 9-month treatment journey of a patient who wore the aligners diligently.

The patient had an anterior reverse bite, with a crowded premolar showing a scissors bite.

The patient first visited us in October of last year for Invisalign treatment.

We decided to treat the cavities visible in the photographs one by one during the Invisalign treatment.

The patient also wanted old amalgam fillings replaced with tooth-colored restorations, so we decided to carry out those treatments along the way.

It is not necessary to complete all cavity treatments before starting Invisalign treatment.

Unless the cavity is extensive enough to require a crown, treatment with either composite resin or an inlay can often be performed during orthodontic treatment.

However, crown treatment may significantly change the shape of the tooth, which could cause the Invisalign aligners to no longer fit properly.

Therefore, it is essential to discuss this matter with your dentist.

This is the result after wearing all 19 aligners in the first set, from November 2023, when the patient began wearing the aligners, through March 2024.

The patient wore the aligners diligently, resulting in a good outcome.

However, because the upper lateral incisor did not sufficiently overlap the lower front tooth and we wanted to further reduce the black triangle between the front teeth, we proceeded with the fabrication of additional aligners.

After waiting approximately one month to receive the additional aligners, the patient wore them diligently for three months, from February through July 2024.

As the additional-aligner treatment was nearing completion, gum contouring was also performed to align the gum line indicated by the blue arrow.

From top to bottom: immediately before gum contouring, immediately afterward, and one week later.

Finally, the treatment was completed this month.

The black triangle was eliminated with the additional aligners.
