Today, I would like to share a case of Invisalign Lite treatment for butterfly-shaped front teeth and gum recession in a patient in their 50s.
Many people need orthodontic treatment but worry that it might worsen their gums.
This is one reason why middle-aged people in their 40s and 50s, in particular, hesitate to undergo orthodontic treatment, even though the same concern applies to young adults.
“ My gums are weak. Will orthodontic treatment make them worse?”
In fact, many patients who visit us for middle-aged orthodontic treatment ask this question.
There is a possibility of this happening if a patient with thin jawbone and receding gums undergoes comprehensive orthodontic treatment lasting one to two years.
The dentist must examine the patient very carefully.
However, with partial orthodontic treatment to straighten only the front teeth, there is not much tooth movement itself, so the likelihood of the gums worsening because of orthodontic treatment is very low.
Of course, many patients mistakenly believe that their gums have collapsed after orthodontic treatment.
This misunderstanding is easy to have when black triangles become larger after treatment.
When teeth have been crooked and overlapping for a long time, straightening them reveals areas where jawbone was originally absent. It does not mean that the gums receded because of orthodontic treatment.

Teeth that protrude significantly from their original alignment often have receding gums.
This is called gum recession.
However, when such teeth are moved back into their proper positions through orthodontic treatment, the receded gums may sometimes recover.
Today, I will focus on this aspect and introduce a case of Invisalign Lite treatment for butterfly-shaped front teeth and gum recession in a middle-aged patient in their 50s.

A patient in their 50s visited us for orthodontic treatment because the front teeth had gradually become more protruded and misaligned.
The patient had already completed extraction-based orthodontic treatment a long time ago, but as they aged, the front teeth gradually became misaligned again, so they came in for partial orthodontic treatment of the front teeth.
After seeing Yeo Esther achieve results with clear aligners, Invisalign, on a television program, the patient found the courage to visit us.
In the end, it was an excellent decision to come in.

The area indicated by the blue arrow shows gum recession compared with the neighboring teeth.

This is because that tooth was the farthest from the proper alignment.
When it is moved back into its proper position, the gum may recover.

The two upper front teeth were protruded and rotated.
Although it is not an official term, patients often call teeth like these “butterfly teeth.”
At this level, Invisalign Lite is sufficient for orthodontic correction.
Invisalign Lite provides a maximum of 14 aligners per set.
Considering the patient’s age and gum condition, changing the aligners every 10 days rather than every 7 days generally places less strain on the gums.
The time required to wear all 14 aligners in the first set was 140 days, or approximately five months.
This is how the teeth looked after wearing all the aligners for five months.

The teeth were aligned, and the black triangles were reduced through minor interproximal reduction.
However, there was a difference in the heights of the upper front teeth, so we decided to address this with additional aligners.

The angle of the protruded front teeth improved, and the butterfly-shaped and rabbit-like teeth were aligned.

The result was already quite good, but we proceeded to make additional aligners with the goal of achieving a perfect result.
The additional aligners were worn for 10 days each for three months, from February through May, completing the treatment.

The black triangle between the upper front teeth was reduced through interproximal reduction, but black triangles appeared as the previously crooked lower front teeth became aligned.
The size of the black triangles in the lower teeth was likewise reduced through interproximal reduction, but this procedure should be performed only to the extent that it does not place excessive stress on the teeth.

Looking closely at the tooth whose gum had receded particularly far because it was the farthest from the proper alignment:

The gum recession recovered, reducing the exposure of the tooth root.
By the time people reach their 40s and 50s, their front teeth naturally become crooked due to the direction of normal bite forces and the natural aging process.
If left untreated, the condition may become more severe.
Please do not give up thinking that it is already too late. Invisalign is a relatively manageable treatment option, so consider consulting a dental professional.
Can Invisalign treatment be performed even in your 50s?
Yes. With a thorough evaluation of the gum condition and an appropriate treatment plan, clear aligner treatment can be performed in middle age as well.
Will orthodontic treatment make the gums recede further?
Rather than being caused by orthodontic treatment itself, black triangles often become noticeable when the existing shape of the jawbone is revealed.
Can gum recession recover?
When teeth that are significantly out of alignment are moved back to their original positions, the gums may partially recover in some cases. However, the same result cannot be expected for every patient.
Why are aligners changed more slowly during middle-aged orthodontic treatment?
Adjusting the rate of tooth movement while considering the condition of the periodontal tissues may help reduce the burden placed on the gums.
When is Invisalign Lite suitable?
It can be a good treatment option when mild alignment improvement focused mainly on the front teeth or partial orthodontic treatment is needed.
