As explained in the previous teenage double eyelid post, many people get scared in advance and choose the non-incisional method, only to end up with a line that becomes too thick—what is commonly called “sausage eyes”—and then come to Ruho Plastic Surgery for revision consultation.

Sausage eyes refer to double eyelids that remain thick, deep, and prominent even after a considerable amount of time has passed since surgery and the swelling has gone down.

Because they can occur for various reasons, let’s look at the appropriate solution for each situation.

As the saying goes, “Know yourself and know your enemy, and you will win every battle.”

To solve sausage eyes, let’s first clearly define what sausage eyes are.

While it may vary from person to person, objectively, a high double eyelid of 8 mm to 10 mm or more,

or a double eyelid accompanied by a very deep appearance or lashes that seem lifted, is called sausage eyes.

For non-incisional surgery, swelling is considered fully gone after 6 months, and for incisional surgery, after 1 year.

Even after this period has passed, eyes that still look swollen as if they were just operated on are also called sausage eyes.

At first glance, sausage eyes, high double eyelids, and thick double eyelids may all look the same.

However, the condition inside the eyelid can be different.

It can be divided into four causes:

1. The double eyelid itself is too large

2. The fixation is incorrect

3. The scar is wide

4. There is ptosis

Now let’s look at the different solutions according to each cause.

1. When the double eyelid is thick

When the double eyelid is thick and has become sausage-shaped, reducing the double eyelid itself will decrease the thickness and improve the sausage-eye look.

If the existing double eyelid line is 10 mm, the principle is to redesign it by reducing it to 7 mm.

An incision is made at the 7 mm size, and the existing double eyelid above it is carefully loosened and removed.

If there is excess skin, the skin in between is excised to remove the scar.

The second case is when the fixation of the double eyelid is incorrect.

This can be divided into two types:

1. High design & low fixation

2. Low design & high fixation

Here, fixation refers to the points used to secure the double eyelid.

If a high design is fixed too low, too much skin remains between the double eyelid line and the lashes,

and the sagging-looking skin covers the lashes, creating sausage eyes.

In the opposite case, the lashes may appear lifted and the double eyelid line can become too deep.

As shown here, even though they are all called sausage eyes, the appearance differs depending on the cause.

As a solution, it can be easily improved by simply changing the position of the fixation points.

The third case is when the scar is wide and makes the eyes look like sausage eyes.

Normally, double eyelid surgery should create a thin, linear adhesion called line-shaped adhesion,

but if too much tissue in the double eyelid line is removed,

a wide double eyelid called sheet-shaped adhesion can form.

In such cases, removing the existing scar and re-fixing the double eyelid can help improve the sausage-eye appearance.

Since scar removal requires cutting out the scar tissue,

the first step is to consult with a specialist and assess the amount of excess skin, skin thickness, and degree of scarring.

Lastly, the fourth case is when there is ptosis.

If the thickness of the double eyelid is normal but the eyes open less because of ptosis,

the eyelids may look thick even if the double eyelid line itself is normal.

If the thickness of the double eyelid is normal, this can be easily solved through ptosis surgery that improves eye opening!

The most important thing in a procedure that reduces the double eyelid is the amount of “excess skin.”

During the process of incising the scar and the skin protruding like sausage,

if there is not enough excess skin, the problem of the eyes not fully closing after surgery can occur.

And in some cases, the skin is extremely thin, the scar is wide, and there is no excess skin.

These cases are quite difficult when it comes to revision surgery,

and depending on the condition, it may be necessary to consider the timing of revision surgery after allowing 충분한 time.
