Hello, this is Luho Plastic Surgery.
These days, many people are coming in for revision surgery consultations.
Some people think lightly about revision surgery,
while others come in with serious concerns and a lot of worry.
To keep you from taking revision surgery too lightly,
and also from worrying too much, we will take a detailed look at questions about revision surgery,
especially cases where revision surgery is not possible.
Q. When is eyelid revision surgery not possible?

A. First, there are cases where the condition of the eyes is not suitable for revision surgery.
Second, revision surgery may be possible, but the result of the surgery may come out differently from the desired result. In other words, the surgery can be done, but if the desired outcome is unlikely, that is a case where more caution is needed.
Q. What are cases where the condition of the eyes is not suitable?

A. It is better not to have revision surgery within 6 months after the initial surgery.
During this period, scars are maturing, so surgery can be difficult,
and in many cases, the problems improve over the 6 months that pass.
If you have had revision surgery multiple times, there are occasional cases where you need to wait not 6 months,
but 1 year or 2 years.
★Cases where revision surgery is not possible★

A. 1. Regarding the condition of the eyes, there are cases where revision surgery cannot be performed.
For example, skin removal may be needed, but if there is no excess skin, the skin cannot be removed.
- If the eyes do not close well to a severe degree, and surgery is performed to make them open wider,
it may worsen the inability to close the eyes and cause functional problems.
In such cases, revision surgery cannot be performed, so do not rush;
there are cases where it is better to wait longer before proceeding with revision surgery.
Q. When is revision surgery possible, but not recommended?

A. First, there are cases where the surgery does not produce the look or shape the patient wants.
For example, if I have a relatively small double eyelid line and have an in-line,
and I want to change it to an out-line, but if turning an in-line into an out-line
does not suit the eye, there may be cases where the out-line looks completely different from the photo the patient brought in
or from the line they want in their mind.
In such cases, surgery is possible, but because the shape after surgery will not be beautiful, we do not recommend revision surgery.
Second, there are cases where the desired design can be achieved with surgery,
but doing so may cause functional problems.
In such cases, we do not recommend revision surgery.
After revision surgery, the shape may come out exactly as desired,
but if it is highly likely to loosen again not long afterward, or if it is predicted that it may cause things like dry eye,
we explain that in advance and, if possible, recommend not having the surgery.
Q. What if the patient desperately wants it, even then no?

A. In actual consultations, when I tell someone that revision surgery is not possible,
some people even get teary-eyed. Emotionally, I empathize with them, but
surgery is medicine and science, so
because some degree of outcome prediction is possible,
if there is nothing that can be improved with revision surgery, or if another functional
problem may occur, it can cause even greater stress than before surgery.
Q. For people considering revision surgery

A. When you come in for a revision surgery consultation, please come with an open mind,
ask detailed questions about the content,
and make sure that after revision surgery, it does not become another wrong decision
and does not lead to yet another revision surgery. I hope it becomes that kind of consultation.
When considering revision surgery, do not just think, I should do it the way I want.
There are cases where that leads to an undesired result and then to another revision surgery.
To avoid that, I hope you will definitely address that point during your consultation.
