Tailored Information for Thyroid Storm

Definition
Thyroid storm refers to a condition that occurs when there is excessive exposure to thyroid hormone, causing overactivation of the sympathetic nervous system and an excessive response.

Triggering Factors
These include undergoing non-thyroid surgery without knowing that the patient has hyperthyroidism, infection, trauma (typically pharyngitis, pneumonia), and childbirth. In addition, iatrogenic causes are as follows.
· Abrupt discontinuation of antithyroid drugs
· Rapid administration of iodine without treatment with antithyroid drugs
· Immediately after radioiodine therapy without pretreatment with antithyroid drugs
- Clinical Symptoms
· Early characteristic symptoms include high fever, rapid pulse, and changes in mental status.
① General symptoms
· High fever · Excessive sweating · Weakness, fatigue
② Cardiac symptoms
· Tachycardia: If thyroid storm persists, rapid arrhythmia may develop and place a burden on the heart.
③ Gastrointestinal symptoms
· Diarrhea · Nausea · Vomiting · Abdominal pain
④ Psychiatric symptoms
· Extreme anxiety · Extreme fear · Confusion
- Treatment
· Remove all triggering factors
· Administer oxygen and glucose-containing fluids
· Treat high fever with external cooling and acetaminophen
※ Aspirin is not used because it has the effect of increasing thyroid hormone levels.
· Beta-adrenergic blockers: treatment for tachycardia, tremor, and anxiety
· Administer propylthiouracil and potassium iodide
· Steroid administration: suppresses the secretion of thyroid hormone
· Administer sympathetic nerve blockers
· Peritoneal dialysis, hemodialysis: reduction of T3 and T4 levels
