This is a case of a female patient in her 60s. She had been considering upper blepharoplasty because of eyelid sagging and hollowing, but due to the nature of upper blepharoplasty, she felt burdened by the incision.

She was looking for a more natural and less invasive alternative. Through the first eyelid fat grafting procedure, she experienced a certain degree of improvement, and after some time, since she was highly satisfied, she decided to consider a second fat grafting procedure. As a result, after the second procedure, the volume above the eyes was filled in more naturally, and the overall impression improved to look softer and more stable. In particular, the survival rate was much more stable than after the first procedure, and the patient herself gave the subjective feedback that “my eyes look less tired.”

Upper blepharoplasty vs. eyelid fat grafting: how to choose?

There are certainly cases where upper blepharoplasty is necessary. However, if the main change in appearance is due to hollowing rather than sagging, restoring volume through fat grafting can provide a more natural and functional improvement. In particular, for middle-aged and older adults who are hesitant about surgery, non-incisional fat grafting is much more preferred, and even with repeated procedures, it has the advantage of fewer side effects.
