A woman in her late 30s, Ms. A, recently found herself becoming more and more bothered every time she looked in the mirror by her increasingly sunken upper eyelids, along with weaker eye-opening strength and a drooping eye shape. She said that the tired impression she used to ignore and the eyes that made her look older had also lowered her confidence. So, through a consultation, we recommended upper eyelid fat grafting together with non-incisional ptosis correction. Let’s go over below why the two procedures were needed together.

Why ptosis correction is needed: strengthening weak eye-opening force
Many people ask, “Wouldn’t fat grafting alone be enough?” But in Ms. A’s case, the levator muscle, which was congenitally weak, became even weaker with age, resulting in hollowing + sagging + insufficient eye-opening strength, which made her eyes look tired.
In cases like this, rather than simply filling volume with fat grafting, non-incisional ptosis correction is needed to adjust the muscle strength, strengthen eye-opening force, and define the double eyelid line more clearly to complete a truly “clear and natural” eye shape.
Through the consultation, we recommended upper eyelid fat grafting and non-incisional ptosis correction together, but here is the important fact! Not everyone necessarily needs ptosis correction. Please note that it depends on the diagnosis, the individual’s condition, and the patient’s needs (desired direction).
Consultation points
Ptosis correction only when needed
And only when desired
Grace is a conscientious clinic that makes decisions after consultation without overtreatment. (Automatic recommendations? No no.)
We do not simply perform ptosis correction and upper eyelid fat grafting. At Grace Plastic Surgery, we provide a variety of combined eye surgeries tailored to each patient, as follows.
Upper eyelid fat grafting: restores volume to sunken upper eyelids
Non-incisional ptosis correction: strengthens eye-opening force for a clearer eye shape
Upper blepharoplasty: corrects sagging upper eyelids
Lower blepharoplasty: improves dark circles through lower eyelid fat repositioning
Lateral and lower canthoplasty: expands the eyes horizontally and vertically
Revision eye surgery: supplements and improves previous surgical results
Medial canthoplasty reversal: restores a natural eye shape by reversing excessive medial canthoplasty
Revision lateral canthoplasty: corrects unnatural lateral canthoplasty

The eyes are a very complex structure on the face. In some cases, it is difficult to achieve the desired change with just a double eyelid procedure or fat grafting alone. A difference of just 1–2 mm in the eyes can completely change one’s impression. That is why our principle is to be more honest and careful, and to suggest only the solutions truly needed by the patient without overtreatment.
