Before and After: Eyelid Fat Grafting Maintained for Over 2 Years in My Late 30s

When the upper eyelids look sunken, a person’s impression can easily seem tired or depressed. Especially after the late 30s, the fat layer in the eyelids naturally becomes thinner and skin elasticity decreases, making it easier for a “deeper-looking hollow” to appear. The case introduced today showed all of those changes at once, and it is an example of how a very delicate, small-volume graft can create a natural recovery.

Because the degree of hollowness differed between the two sides, slightly different amounts were used. Even though this was a small-volume graft, the skin on the upper eyelid is thin, so if it is placed accurately in the correct layer, the overall impression can change significantly. The initial response was also good. The shadow that made the eyes look tired disappeared, the tension felt when opening the eyes eased, and the gaze became much softer. A second graft was performed 4 months later. The especially important point in this case was the second autologous fat graft. With fat grafting, about 70% of the graft typically takes during the first season, and then very subtle differences can occur depending on eyelid movement and lifestyle patterns. In this patient, the initial take was very stable, but a reinforcement graft was performed at 4 months to achieve more perfect left-right balance and smoother contours. The reason the second graft was beneficial was that the first fat had already settled well, creating a stable supporting layer, and only the necessary areas were supplemented very thinly to complete a smooth eyelid line. While maintaining naturalness without being excessive, the second graft also helped improve the take rate further. Because the total amount used was not large, the natural look was preserved without any obvious volume.

This is a case with excellent long-term retention and graft take. Even in the photos, the upper eyelids maintain almost exactly the originally designed eyelid-hollow line, without an uneven surface, sagging fat, or clumping.
