After the mid-40s, hollowing in the upper eyelid crease area and the upper eyelid itself often progress at the same time, making the eyes look tired and the overall impression suddenly appear older.

In this case, orbital fat injection and upper eyelid fat grafting were performed together to restore volume in the hollow upper eyelid crease area and naturally fill the sunken area above the eyes.

By injecting only the amount needed to match the eye structure, without excessive volume, the result improved into a comfortable and defined impression without unnatural fullness. At the 3-month point, fat retention had stabilized, and the sunken upper eyelid area and upper eyelid crease shadow had been reduced.

The reason we disclose many before-and-after photos is to clearly show the range of results. Because outcomes can differ even with the same procedure depending on eye shape and lifestyle habits, we use a variety of cases so that it is possible to objectively see what changes appear under which conditions. This is part of providing the information patients need to understand and evaluate the procedure. Upper eyelid fat grafting is not simply a procedure that fills in hollow areas. The area above the eyes has thin skin and a complex structure of muscle and fat layers, so an accurate understanding of the anatomical structure is necessary. Depending on the grafting location and depth, the naturalness and stability of the result can vary greatly.

At Grace Plastic Surgery, we select and use only fat with uniform particle size from fat harvested from the thighs or abdomen. By removing impurities as much as possible, we focus on improving retention and reducing the possibility of side effects such as swelling or lumpiness. Because the upper eyelid crease area can create an unnatural bulge or a thick appearance if too much is injected, micro-layered grafting is performed in 0.1 cc increments. It is important to inject exactly the needed amount while taking into account the eye structure and the movement of the orbicularis oculi muscle. In upper eyelid fat grafting, location and layer are more important than volume itself, and a precise approach based on understanding the anatomical structures around the eyes and clinical experience makes the difference in results.
