A board-certified plastic surgeon specializing in forehead lifts answers.
Q. Which should I choose, Endotine fixation or thread fixation?

A. At our clinic, we mainly use Endotine.
We recommend Endotine to our patients. However, because Endotine is an expensive material,
in some cases, if a patient prefers thread fixation depending on the situation, we also perform forehead lift surgery using thread.
Many people are also curious about the difference between Endotine and thread.
Endotine has five spikes, and the distance between the spikes is 1.5 cm.
During surgery, two points can be adjusted: the inner point and the outer point of the Endotine,
for example, when fixation is done with thread, one point is involved.
Because Endotine has five spikes, the force can be distributed across five areas, and the two points are engaged at 1.5 cm intervals.
So if you want to pull the outer area a little more,
the pull points can be adjusted differently, which may lead to higher satisfaction.
Q. Forehead lift: 3 incisions vs. 5 incisions??

A. Sometimes patients who come for consultations ask whether a forehead lift clinic that makes five incisions lacks skill.
On the contrary, if the procedure is done with 3 incisions or 2 incisions, it can be difficult to widen the dissection area.
Using 5 incisions makes it possible to achieve a wider dissection.
Of course, a forehead lift can be performed without dissecting as far as the area around the eyes,
but performing the dissection over a wider area can lead to more satisfactory results, and the duration of the effect may also be longer.
Q. After the duration of a forehead lift, does it return to how it was before?

A. When the duration of a forehead lift is 5 to 10 years, it does not mean that you return to your pre-surgery state;
rather, the starting point changes.
If the forehead skin has been lifted through a forehead lift, the skin will gradually come down over 10 years.
If you had not had a forehead lift, it can be seen as having descended further than before the forehead lift.
Also, if the dissection is performed accurately during a forehead lift, the duration may be longer.
Among the patients who came to see me for forehead lift revision surgery, there were some who had undergone a forehead lift but had almost completely drooped back down after one year.
In such cases, it can be said that the dissection was not performed accurately.
Also, regarding whether forehead lift surgery needs to be revised periodically,
patients who had a forehead lift at another hospital but experienced little effect or were dissatisfied because it came down too quickly
end up undergoing revision surgery.
Q. How many times can forehead lift revision surgery be performed?

A. Forehead lift revision surgery is certainly possible, and there is no major limit on the number of revisions.
For example, with double eyelid surgery, people may have it once or twice in their lifetime, but there are also many who have it five or six times, or even more.
There is no limit on the number of forehead lift revisions, but
if the initial forehead lift was performed with sufficient dissection and a systematic plan,
there would not be a need for multiple revisions in one lifetime.
