Comparison of Incision Double Eyelid Surgery and Non-Incision Double Eyelid Surgery
What is double eyelid surgery?
A cross-section of the eyelid is arranged from the outside inward as skin, muscle, soft tissue, orbital fat, the muscle that opens the eye, and the tarsal plate.
A double eyelid is formed when a connection is made between the eyelid skin, the tarsal plate, and the muscle that opens the eye, causing the skin below that connection to be pulled upward under the upper eyelid when the eye is opened.
Double eyelid surgery is a procedure in which the skin below the line is connected to the fascia or tarsal plate to create an artificial crease.
Depending on whether the skin is incised, it can be divided into the buried suture method (non-incision method) and the incision method.
- Buried Suture Method Non Incision Double Fold Eyelid
This is a procedure that is preferred because of the short recovery period and the absence of surgical scars.
It is possible for young patients with not too much fat and little skin sagging who do not want visible surgical marks.
However, its biggest drawback is the very high possibility of the crease coming undone, so revision surgery should be kept in mind. As aging progresses, the line can easily loosen or become deformed, which is why it is also a procedure mostly preferred by people in their teens and 20s.
This procedure may not be possible depending on the thickness of the eyelid skin or the structure of the eyes. Rather than unconditionally preferring a specific surgical method, choosing the method according to your own eye condition can lead to better results.
It is important to choose the surgical method through sufficient consultation with an experienced board-certified plastic surgeon.
Usually, about 3 weeks after surgery, the tension of the suture eases and it gradually looks more natural.
If it is revision surgery, incision surgery is definitely recommended.
The buried suture method cannot be applied to cases with severe eyelid sagging, a lot of fat, thick skin, or to patients in middle age or older.

- Incision Method Incision Double Fold Eyelid
The incision method is a procedure in which the double eyelid line is incised and then an appropriate amount of the various tissues in the eyelid, such as skin, muscle, fat, and connective tissue, is removed and sutured. When the dermal layer of the skin below the double eyelid line is sutured to the tarsal plate and adhesion is created between the tissues, the double eyelid line is formed accurately and clearly.
Compared with the buried suture method, the incision method is less likely to come undone and has the advantage of being able to create a definite double eyelid line depending on the fixation method. However, swelling may last relatively longer after surgery, and it may take some time before it looks natural.
If the eyelid sagging is severe, an upper blepharoplasty procedure that removes the excess sagging skin may additionally be necessary.
By understanding the patient’s eye structure and according to the patient’s needs, the height of the double eyelid can be raised or lowered, and the design can be adjusted. Sufficient discussion before surgery is recommended.
