There is no revolutionary surgical method; the goal of revisional surgery is only to reduce side effects and delay recurrence. In some eyes, lower eyelid retraction can occur easily, and especially due to previous surgery, scar tissue, adhesions, and retraction (a state in which the eyelid is pulled downward or turned outward) may develop. The patient had lower blepharoplasty once in the past, but it recurred again, with under-eye fat sagging and causing fullness beneath the eyes, along with sagging of the muscles and skin. Inside the lower eyelid, there is scar tissue and internal adhesion of the muscles due to the previous surgery, which makes the operation more complex. Because of the earlier easy-to-access method (fat removal), proper repositioning was not achieved, so the upper eye fat sags and the upper eyelid appears sunken as well. In revisional surgery, for these reasons, anesthesia may not work well or dissection may be difficult.

Surgical method and recovery process for the incision site in lower blepharoplasty

• An incision is made just below the eyelashes along the wrinkle direction to secure a sufficient opening.

• After reaching the fat layer through dissection, the fat is repositioned. In revisional surgery, repeated fat removal and grafting may have caused the tissue and fat to adhere, so the procedure can be difficult.

• Reinforcement of the orbital septum, suprasmooth fat lifting, and muscle reinforcement preserving the lower eyelid muscle are performed.

• The midface muscles are pulled and fixed to the periosteum.

• The loosened skin is removed, pulled tight, and sutured.

• The more loose skin is removed, the more the pulling-down force also acts.

• Therefore, removing too much indiscriminately can cause a side effect of insufficient skin. Insufficient skin can have a poor prognosis to the point that revisional surgery becomes difficult.

• Not all wrinkles can be smoothed out like ironing. Rather than completely removing wrinkles, it is more natural and safer to be somewhat forgiving of fine wrinkles.
