Why Under-Eye Bulging Happens
Have you ever felt that the area under your eyes looks puffy and shadowed every time you look in the mirror or take a photo?
When fat builds up and bulges under the eyes, it can create shadows and make your expression look less pleasant.
If this makes you look older and causes appearance-related stress, you may want to consider under-eye fat repositioning.
Bulging under-eye fat can be described as the result of the orbital septum, which surrounds the eyeball, losing elasticity due to aging and other factors, causing the fat around the eye to slide down and accumulate under the eyes. When the fat protrudes, it creates shadows, dark circles, and tear troughs, changing your overall mood or impression.
This protruding fat pouch is called an eye bag.
Among various causes, aging is the biggest one, but not everyone develops it just because they get older.
Some younger people naturally have more fat, and it can even appear in the teens.
As people age, the orbital septum stretches relatively faster, and if the eyelid skin is thin, it may stand out more.
Learn When Under-Eye Fat Repositioning Surgery Is Needed
-
When orbital fat pushes outward and the under-eye fat protrudes excessively
-
When the under-eye area is sunken and looks dark because blood vessels show through
-
When wrinkles are layered under the eyes, making you look older
-
When the under-eye area is uneven

Ahn Sungmin, Plastic Surgery Specialist
Excessive fat removal can cause the surrounding fat to be lost as well, leading to a sunken under-eye area.
Once removed, fat does not regenerate, so the goal at Ahn Sungmin Plastic Surgery is not to remove it, but to reposition it and restore volume.
Even when there is too much under-eye fat and removal is necessary, excessive removal can cause side effects such as hollowness in the upper eyelid.
For middle-aged patients or when under-eye skin sagging or wrinkles are present, lower blepharoplasty is performed to remove loose skin, along with fat repositioning surgery.

Midlife Under-Eye Fat Surgery
During under-eye fat repositioning surgery, recurrence can be prevented by strengthening the orbital septum and improving its support.
As aging progresses, the overall structure beneath the eyes weakens and loosens, so over time the repositioned fat may push outward again.
Therefore, by reinforcing the orbital septum, we strengthen its support so that the fat does not push out again.
The surgical method differs from one clinic to another. Surgical techniques are evolving toward simpler procedures, but these are not suitable for middle-aged patients.
For middle-aged patients, surgery should not be a temporary improvement; it should be the final one.
In particular, if there are many wrinkles and severe fat sagging, an incision is necessary to prevent recurrence. As you get older and aging progresses, gravity causes the fat to sag more and the skin to stretch. Therefore, the patient’s condition must be diagnosed and the most appropriate surgery planned and performed.
Simple procedures done through the conjunctiva or without an incision are temporary and cannot fundamentally improve sagging skin.
For middle-aged patients, surgery is not something to try like a trend. Rather than a simple method, we recommend the most fundamental, classic method that does not recur.
