Why is fore front surgery necessary?
The average horizontal width of East Asian eyes is about 28–29 mm, and when it is around 31–32 mm, the eyes look more open and refreshed.
Inner canthoplasty is commonly called fore front surgery, and it is a surgical method that increases the horizontal length by naturally expanding or correcting the front part of the eye.
By removing the Mongolian fold at the front of the eye or correcting its height difference, the tightly covered area can be made to look more open.
It is also sometimes performed together to naturally connect the double eyelid line.
If there is an epicanthal fold (Epicanthal fold), the front of the eyes can look tight and the distance between the eyes can appear wider.

Cases where fore front surgery is needed
-
When you want to extend the horizontal length of the eyes toward the front so they look more open.
-
When the distance between the eyes is wide.
-
It is needed during double eyelid surgery to create a stable extension of the line.
Precautions during fore front surgery
-
How much should be exposed? The amount of lacrimal caruncle exposure must be considered.
-
Exposure should be determined by considering the distance between the eyes, and the shape and color of the lacrimal caruncle.

Before and after inner canthus correction surgery
The degree of lacrimal caruncle exposure varies greatly from patient to patient.
If too little is visible, the horizontal length may be short and feel cramped; if too much is visible, the impression may look strong and older.
The appropriate degree of exposure differs depending on the shape that is exposed, and the design also changes, so please consult a board-certified plastic surgeon.
If, when pulling the skin, the shape of the lacrimal caruncle is hook-like or not aesthetically pleasing, it is better to expose less.
The following are before-and-after photos of fore front revision surgery.

Goals of fore front surgery
- Scarring should be minimal. Fore front scars are in the central part of the face, so they can become a source of self-consciousness.
Scars must be minimized.
- The inner canthus should connect naturally with the double eyelid line so that the overall shape looks natural.
Before and after fore front surgery
Because the pull of the Mongolian fold band is released, the vertical width changes depending on the degree of tension.

Possible side effects of fore front surgery
Undercorrection and overcorrection (excessive exposure of the lacrimal caruncle)
Melanin pigmentation
Surgical scar such as bulging or indentation of the inner canthus
Double folds at the inner canthus
A feeling of being pulled inward
Recovery process after fore front surgery
The recovery process after fore front correction differs from person to person.
In general, if only fore front surgery is performed, recovery occurs within one month, but in most cases it is performed together with double eyelid surgery.
After about 6 months, it settles naturally.

Surgery time and care
The fore front surgery time is about 30–40 minutes for both sides combined,
and it is performed under local anesthesia.
Usually, it is performed together with double eyelid surgery to maximize the effect of the double eyelid procedure.
For the first 1–2 days after surgery, it is better to rest and avoid overexertion.
After surgery, for about 2 weeks,
Do not irritate the eyes by rubbing them hard.
In particular, fore front surgery is closely related to scarring and is a part that is often touched because of eye discharge, so care must be taken to avoid irritation.
