Double Folds Caused by Multiple Surgeries
After double eyelid surgery, multiple folds can sometimes form on the eyelid. An ideal double eyelid is a single smooth line, but if the eyelid is thin or there is significant bulging, the eyelid may fold like an accordion when opening the eyes, creating multiple folds.
In revision surgery cases, the scar from the existing double eyelid line may fold and appear as multiple folds. When repeated surgeries cause internal tissue adhesion and hard scar tissue to build up, multiple folds may appear instead of a natural line.
Evaluation Based on the Line Seen When the Eyes Are Open
Based on the line visible when the eyes are open, the condition of the double eyelid can be divided as follows:

A natural and stable double eyelid does not have the line directly exposed on the eyelid when the eyes are open, and the ideal line is one that forms naturally as the eyelid comes down.

The patient below is a case in which all scars from four previous surgeries are still visible. In a normal double eyelid, the scar line is visible only when the eyes are closed, and when the eyes are open it naturally folds inward and becomes one with the eyelid. If the scar line remains visible when the eyes are open, it means the internal eyelid tissue was not properly organized due to previous surgeries.

Revision Surgery Case at Ahn Sung Min Plastic Surgery
This patient had not had ptosis corrected properly, and only the double eyelid line had been made excessively large. As a result, when the eyes were open, all previous surgical lines were exposed, and a natural eye shape could not be achieved.
After observing the patient’s eye-opening habit, it was found that due to severe ptosis, the patient was using the forehead to open the eyes, which caused the eyebrows to rise and all existing lines to become visible. Therefore, to address the fundamental problem, ptosis correction was performed together.

Severe Adhesions Caused by Multiple Surgeries
Scars from the previous surgeries remained inside the eyelid, and the patient had severe internal adhesions. This patient was undergoing a fifth eye revision surgery case, and due to an unnatural and strong impression, was also experiencing difficulties in interpersonal relationships.
Among revision surgery cases, this case is particularly difficult, and compared with typical eye revision surgery, the difficulty, time required, and cost were all significantly higher.
During the operation, all adherent tissue was carefully released through an incision. However, because the amount of skin between the eyebrow and the eyelid was limited, it was impossible to completely remove all scar tissue. Accordingly, we loosened the bound tissue and skin as much as possible and reconstructed it into a single stable double eyelid line to create a natural and balanced result.
Why Eye Revision Surgery Is Difficult
Eye revision surgery is one of the most difficult procedures even for an experienced board-certified plastic surgeon.
There is no fixed formula or single correct answer, and the outcome varies greatly depending on the individual patient’s condition, previous surgical history, the anatomical structure of the eyelid, and the degree of tissue damage.
Therefore, only a surgeon with extensive experience in complex revision cases and precise judgment can determine the safest and most appropriate surgical method.
In addition, every surgery must follow a proper sequence, and it is impossible to perfectly fix a poorly performed surgery in a short period of time.
