There are many reasons for nose revision surgery,
from A to Z.
Among them, we selected six reasons
by gathering only the ones people are most curious about.
For those who are considering nose revision surgery
and have questions about revision surgery,
and for those who have many thoughts before coming to the clinic
for an in-person consultation!
I hope this will be helpful to everyone.
Q.1
Is it okay to have the revision surgery done at the same hospital where I had the first surgery?
When revision surgery is not something I choose myself, such as in cases of inflammation or contracture,
the doctor who performed the surgery knows best,
so I think it is better to have the surgery at that same hospital.
However, if you are considering revision surgery because you are unhappy with the appearance,
you should think about whether you trust that hospital.
When you had your first surgery,
“I wanted it to be done this way, but I couldn’t say so,” or
“I thought this would suit me,
but now that I’ve had it done, I don’t think it suits me.”
If your mind has changed,
or if you achieved the goal you had in mind
but want to change the shape again,
I think it is better to have the surgery again with that doctor.
This is because they know your existing condition well
and I believe they can realize it best.
However, if you clearly conveyed your intentions,
but they were not reflected in the result,
then even if you make another request, it may be difficult to expect
that it will be achieved clearly.
In such cases, I recommend consulting another hospital.
That said, when you consult another hospital,
the best thing to do is obtain your surgical records from the hospital where you had the original surgery
and bring them with you.
If that is not possible,
clearly tell the next doctor when and where you had the surgery,
what kind of procedure it was,
what materials were used,
what you do not like now,
so that you can achieve a good result.
Q.2
Can’t the materials used in the first surgery be reused?
First, for silicone used in the bridge of the nose,
they are not reused.
The bridge and the tip of the nose are connected,
and if the position of the tip changes, the implant that was originally inserted
may no longer match the tip accurately.
In most cases, because the shape has changed,
they often do not reuse it unnecessarily.
Even if that were not the case,
there may be a risk of infection or the implant itself may have had a problem,
so I think replacing it with a new implant is a slightly safer approach.
The second case is reusing cartilage.
Cartilage is used for two purposes.
One is to create a pillar-like structure,
and the other is to use it as a decorative material to refine the detailed shape after the structure has been completed.
In the case of cartilage used for decoration,
if there is a shortage of cartilage or if the cartilage is in good condition,
it may be used again in the next surgery.
However, cartilage that was used for structure
is usually needed when the structure must become stronger,
so there is actually no way to reuse a small pillar and turn it into a larger pillar.
That is why it is often replaced with stronger or larger cartilage.
However, in very special cases,
if the height needs to be lowered
but the cartilage itself is very strong,
there are cases where the position or shape of the cartilage is slightly adjusted
for correction.
Q.3
Can revision surgery make the nose much prettier?
This should be judged by two factors.
First, whether the direction you want is possible.
For example,
if you want the tip of the nose to be higher,
you need to consider whether it can actually be made higher.
Second, whether that direction is aesthetically right.
If you are considering revision surgery,
the chances of getting a prettier nose are high,
but if you present a more specific and clear goal during the consultation,
it should be possible to judge whether it can be achieved
and whether a satisfactory result is possible.
If it is not possible, or if it would not look more beautiful aesthetically even if it is possible,
then that approach cannot be used.
If you are expecting the answer, “Yes, this can be done. It will get better!”
please keep in mind that you must go through a consultation
and see the condition in person before I can explain the possibility.
Q4.
How many times can revision surgery be done?
There are cases of re-revision surgery, and re-re-revision surgery after revision surgery...
These kinds of cases do happen.
There are also people who have had nearly ten surgeries.
But in those cases,
it seems that quite a lot of problems occurred in between.
I was dissatisfied,
inflammation or contracture occurred,
and then the situation was not favorable,
so the shape became poor,
...
As they kept trying to solve this,
they ended up having repeated surgeries...
At first, I liked it being high,
but then, “Ah, maybe this is too unnatural.”
...
It was lowered, and then the desire to raise it again came back.
...
So, in cases where revision surgery is done because of an unwanted complication,
solving that problem comes first.
And to prevent complications,
thinking about safer materials and safer surgical methods
seems more important than the desire to improve the shape beautifully.
If revision surgery is being done because of a change of mind,
it is naturally necessary for the doctor to tell you that the direction is not good,
and from the patient’s perspective, I think it is necessary to be more careful in deciding on revision surgery
and to make efforts so that you do not change your mind again.
Q5.
If it has been a long time since the surgery, is revision surgery more difficult?
It is actually easier.
If inflammation occurs and is left untreated for a long time,
it can be bad,
so in such cases, the sooner it is resolved, the better.
However, in cases of dissatisfaction with the appearance,
it is better to think carefully before having surgery.
If you approach it too quickly and hastily,
the surgical result may actually be worse,
and revision surgery after a long time may actually be better.
This is because after surgery,
there are stages of wound healing called the inflammatory stage, proliferative stage, and remodeling stage.
The longer it has been,
the more likely it is that revision surgery will be performed after the wound has gone through the final stage of remodeling,
so the surgery is easier,
and the likelihood of complications or side effects is lower.
So, doing it too quickly is not good,
and I would like to say that taking your time to think it through may actually lead to better surgical results.
Q6.
Is there anything that needs extra attention during revision surgery?
Strictly speaking, there is nothing that specifically requires extra attention,
but of course thorough management is necessary for the best results.
In revision surgery, recovery may be delayed.
Therefore,
in order to prevent unsatisfactory results caused by insufficient aftercare,
it is important to avoid pressure after revision surgery
and not to start strenuous exercise early.
