When it comes to surgery, anesthesia is
something many people wonder about:
how it is performed
or whether it hurts.
Anesthesia can be broadly divided into
local anesthesia💉
depth sedation💉
general anesthesia💉.
In the past,
even simple eye surgeries or nose surgeries
were often done under local anesthesia.
Local anesthesia only numbs
the specific area,
so you have to get injections several times.
For double eyelid surgery,
you may need about ten stingsx10,
so it is no small ordeal.
It can be painful.
However, these days,
depth sedation is often used
to reduce pain.
That said,
if you do not want it,
it is not done.
With depth sedation,
there is almost no pain other than
one injection in the arm.
You do not feel pain at all during surgery.
Then does depth sedation
mean you just keep sleeping?
No!
It starts with depth sedation,
and once the area is numb, you are awakened.
That is because they need to check
the size and shape of your eyes when open.
There is no need to keep you asleep for long,
and if it does not hurt, there is no need to keep you asleep.
Sometimes during surgery,
some people say they are afraid of waking up.
But once you actually go through it, it is not that scary.
Some even ask us to scratch their nose
because it itches😅
Some people have a hard time after depth sedation,
and there are two cases.
Either the anesthesia did not work well and it was difficult.
Or the anesthesia worked too well and it was difficult.
If the anesthesia does not work well,
and you wake up during surgery or feel pain,
that means too little anesthetic was administered.
If you feel unwell from the aftereffects of anesthesia,
that is because too much anesthetic was administered.
If too much anesthetic is given,
hangover-like symptoms can be severe.
It is like the day after drinking a lot of alcohol.
Local anesthesia basically has a set duration
once it is injected into the body.
The basic approach is to start numbing the area
and finish the surgery as quickly as possible
before the anesthesia wears off.
When using depth sedation,
it should be like, "I dozed off for a moment and woke up,
and after that it did not hurt."
Sometimes people who are very frightened
ask to be kept asleep the whole time,
but that is not a good idea.
Sedatives for depth sedation are like alcohol.
If you use too much, the hangover will be severe.
Most surgeries are completed within an hour,
so there is no need to make the anesthesia especially strong.
The dosage of anesthesia is basically calculated
based on body surface area.
If you are tall or weigh a lot,
the anesthesia may feel weaker.
Also, most anesthetics are metabolized by the liver.
People who drink a lot of alcohol
tend to have more active liver function,
so anesthetics tend to break down faster.
That is why
we check weight and height before surgery
and also ask about your usual alcohol intake
when deciding on the anesthetic dose.
If you have anesthesia often, you can build tolerance.
Just as your alcohol tolerance increases if you keep drinking,
the same applies to anesthesia.
If you have undergone depth sedation several times
within the past six months,
it may not work as well.
There is also a superstition that frequent depth sedation
makes your brain worse,
but
that is absolutely not true.
Having anesthesia once or twice a year
for health checkups will not make your brain worse.
Some people also say that having anesthesia during childbirth
made their memory worse.
In fact, childbirth itself is an extraordinary event.
Pain on a 10-point scale is the pain of childbirth,
and when you are feeling that level of pain,
brain cells die.
Giving birth, regardless of anesthesia,
is something that can cause memory to fade a bit more.
Most of the surgeries I perform
can be done with depth sedation.
However,
for major surgeries such as removing ribs
or shaving down the cheekbones,
or when multiple areas must be operated on at once,
general anesthesia is used.
Depth sedation means the patient does not remember,
but is sleeping while breathing on their own,
and general anesthesia means you do not breathe,
so a tube is inserted into the airway
and a machine supplies oxygen.
In fact, general anesthesia can be safer
than regular depth sedation.
Anything can be dangerous
if done improperly.
