The line connecting the forehead and the nose
has a major impact on the nose’s overall impression.
When the nose is high,
the heights of the forehead and nose connect as one,
and it can look as if it starts from above,
like a so-called “lion nose” or “Avatar nose.”
For most people,
it is much more natural for the nose to retreat somewhat
from the most prominent point of the eyebrow
and then project forward again.
In particular, for women,
the starting point of the nose, meaning the most recessed point,
should ideally be located at least around the level of the eyelashes or the double eyelid.
If it rises higher than that,
the nose can look long
and give an artificial impression,
so recently there has been a tendency
not to prefer a starting point that is too high.

A problem often mentioned in revision cases is
“the outline of the implant showing through.”
The most common cause is the use of
an implant that is too high.
If it is excessively high,
the outline can show through no matter what method is used.
However, sometimes even when the implant is not high,
the outline can show through
because the skin is too thin.
For people with thin skin,
the surgery is performed in a way that supplements the skin
and wraps around the implant’s outline.
A representative material used for this is
the temporal fascia.
Recently, surgeons also prevent this issue
by placing a piece of artificial dermis,
called donated dermis,
over the implant.

When seen from the front,
the nose often looks awkward because it appears
“pinched.”
The pinched appearance can happen
because the implant is visible,
but
it often occurs because the width is reduced too much
in a place that should not be narrowed.
A beautiful nose should have a line that extends from the tip
gradually widening outward
for it to look natural.
If it suddenly bends in the middle
and comes inward,
the line below it will look pinched.
In particular, trying to correct a bulbous nose
by narrowing the cartilage spacing too much
can also be a problem.
In reality, there are many cases where the cartilage itself is small,
but the skin is thick,
which makes the nose look bulbous.
In such cases, reducing the cartilage
does not correct the bulbous nose;
instead, it only makes the pinched feeling more pronounced.
I think it is better to perform surgery
in a way that increases the support of the cartilage.

Lastly,
if too much scarring remains,
it can make it obvious that surgery was done.
In such cases, it can be reduced through laser treatment
or scar revision surgery.
The condition where too much of the nostrils are visible,
like a turned-up nose,
may be a “surgical error”
caused by trying too hard to lift the tip of the nose
in the first operation.
Or, after surgery,
contracture can cause the nose to gradually lift upward.
For a turned-up nose caused by contracture,
the cause of the contracture must be addressed,
and tissue that has already shortened needs to be lengthened.
Then, to prevent contracture from happening again,
the nose is corrected by providing support with strong cartilage.
Please remember that a natural-looking nose is not simply a high nose,
but a harmony of very subtle elements,
such as the curve, the starting point, and a line without pinching.
