Three years of hesitation before coming in: the story of a 34-year-old woman

A 34-year-old woman came to see me last autumn. As soon as she sat down, she said:

“My yellowish teeth keep bothering me about the blind dates I’ve finally arranged.” After hesitating for a moment, she added, “But I’ve been too afraid of having my teeth shaved, so I haven’t come in for three years. I heard that once they’re shaved, they can never be restored.”

I understand how she felt. Teeth do not grow back. So, without saying anything first, I scanned her front teeth with an intraoral scanner and displayed them on the screen.

Then I asked her:

“Is what concerns you most the color, the shape, or the gap between the teeth?” She looked at the screen for a while before answering, “The color is the biggest issue, and my two front teeth are also slightly spaced apart.” At this point, the treatment direction is already about halfway determined.

Veneers are made by bonding a thin ceramic shell to the front surface of a tooth. It may be easier to understand if you think of applying gel over a fingernail. The difference, however, is that you need to create a very shallow space equal to the thickness of the shell being placed. If you simply put it on without creating that space, the front teeth may look unnaturally protruded.

The expression “shaving the teeth,” which worries people looking into front teeth veneers in Gangnam, actually refers to the process of creating this space.

What matters is the depth.

Enamel, the outermost layer of a tooth, is usually around 1 mm thick. Smoothing the tooth within this layer is entirely different from extending into the dentin beneath it.

Dentin contains tiny tubules connected to the nerve, so if it becomes exposed, sensitivity may persist.

That is why, during consultations, I always draw this boundary on the screen and explain it carefully.

Three options based on how much the tooth is reshaped

The first is an approach that requires no reduction. A thin shell is bonded with almost no reshaping of the tooth surface, making it suitable when the natural tooth is positioned farther inward or is relatively small.

The greatest advantage is that the tooth structure is left intact.

The second is minimal reshaping. About 0.3–0.5 mm is carefully adjusted within the enamel to create space. This approach offers a good balance when addressing both color and shape.

The third applies when the teeth protrude significantly or overlap and require more reshaping. In such cases, I may recommend partial orthodontic treatment before veneers. If there is a way to reshape less of the teeth, my principle is to show the patient that option first.

The answer I gave her

I recommended the minimal-reshaping approach for her.

The gap was about 1 mm, which could be filled by adjusting the width of the shells, and the color could be controlled through the shell’s thickness and material. We only began treatment after placing a model created from the scan data in her mouth so she could see the expected result for herself.

I now see her for regular checkups every six months. At her last visit, she smiled and said:

“I no longer cover my mouth when I take pictures.” I believe those words best show the purpose of treatment with front teeth veneers in Gangnam.

Q1. Is it really possible without shaving the teeth?

A. It depends on the condition of the teeth. If the natural teeth are positioned inward or are relatively small, treatment may be possible without reduction. If they protrude forward, however, minimal reshaping may be necessary. This can be assessed in advance using scans and photographs.

Q2. Can the teeth become sensitive after the veneers are bonded?

A. No.

The lifespan varies depending on the material and how well they are maintained, but at some point they may need to be repaired or replaced. Habits such as biting hard foods with the front teeth or grinding your teeth can bring that time closer, so we may also recommend a night guard.

Q5. I’m worried about side effects.

A. If a gap develops along the bonding surface, cavities may form in that gap, and the gums may also become red. That is why we examine both the bonding surface and the gum line during regular checkups.

I believe that fear does not mean hesitation or regret; it reflects how much you value your natural teeth.
