To completely eliminate the blunt impression caused by the wide alar base, we first removed the materials used previously, replaced the silicone in the nasal bridge, and used both donated rib cartilage and ear cartilage at the nasal tip to raise it slightly and improve the bulbous appearance.

In addition, we further performed bulbous nose correction using alar reduction, nostril lowering, refinement of the nasal bridge line, and correction of a recessed columella scar, creating a more natural straight-but-soft line and showing an improved impression before and after bulbous nose surgery.

2. Bulbous Nose with a Hump and Nasal Bridge

This patient had little midface volume and had both a hump and a bulbous nose at the same time, so they visited our hospital to improve the hump and bulbous nose and to restore overall volume to the face.

To make the midface appear fuller, the surgery was designed with a refined straight-but-soft line. First, the hump was corrected by moving the nasal bones outside the nose through lateral osteotomy to narrow the width at the base of the nasal bones and straighten the line that appeared bent due to the hump. The hump on the nasal bridge was also addressed to neatly correct the bridge. In addition, for the part where a low nasal bridge made the nose look bulbous, bulbous nose correction and columella lowering were performed to define the overall line of the nose. Silicone was used in the nasal bridge, and ear cartilage and septal cartilage were used together at the nasal tip to raise it and connect it smoothly to the bridge, showing a nose with the bulbous appearance and hump removed.

3. Bulbous Nasal Tip

This patient had a crooked nose from the front, a pinched nasal tip, and a bulbous area above the tip, and visited our hospital to improve the crooked nose and bulbous nose.

After examining the inside of the nose, the deviated part of the septum had not been properly corrected, so the septum was realigned correctly. The nasal cartilage was then strongly sutured to release the pinched appearance and supplement the pinched area and bulbous nose. The bulbous area above the tip was a type that looked blunt because the skin at the nasal tip was thick and there was a lot of subcutaneous fat, so the tip cartilage was sutured again for support. At the same time, some subcutaneous fat was removed, and ear cartilage and donated rib cartilage were used together to raise the tip and make it more refined, showing an improved impression before and after bulbous nose surgery.
