Today, I’d like to talk about a case involving a patient who had complex nose surgery (bulbous nose, crooked nose, and hump nose), was unhappy with both the side profile and the front view,
and had previously injured their nose, leaving the nasal bone slightly crooked and creating a stuffy-looking impression.
I’ll begin this review post about how the surgery was performed in a straight-line style to balance the overall facial features and transform the patient into a model-like image.

Preoperative history / desired image
This patient had previously experienced a nasal bone fracture due to trauma, which caused a crooked nose shape and breathing problems.
They also had long-standing concerns that the overall round and blunt appearance of the nose made their face look stuffy.
In particular, the patient wanted to improve the bluntness of the nasal tip,
and rather than setting a specific line in advance, they preferred to trust and follow the recommended design suggested by the specialist.
Surgical plan


Overall, the nasal skin was not very thick, and from the front, the nostrils appeared to flare outward.
Since the nose had a bulbous shape and a hump on the bridge, we designed the rhinoplasty plan as a straight-line style while taking these features into account.

Nasal tip: 56 mm Nasal bridge: 23 mm
Columella correction + bulbous nose correction + crooked nose correction + columella lowering + nostril lowering
Surgery preparation




After the final design plan is completed before surgery, we check the design with the patient and then proceed with the operation.
Step-by-step changes after nose surgery

Day 2 after surgery
The patient visited the clinic for treatment (disinfection) and removal of the packing.
There was almost no major bleeding or bruising, and we recommended managing swelling with cold compresses during the early stage.
Day 7 after surgery
The sutures and splint were removed at the same time.
At this stage, combining professional swelling management such as laser and injections can help speed up recovery.
1 month after surgery
Most of the major swelling had subsided and the recovery phase had begun, allowing makeup and daily life to resume.
However, some residual swelling may remain, so consistent care is necessary.
Postoperative result changes
Before surgery, the patient had difficulty breathing due to the fractured nose.
After surgery, the breathing function improved, and the patient feels much more comfortable.
Aesthetically, the nose gained more definition from both the side and the front,
and the blunt nasal tip from the front—the patient’s biggest concern—changed to a much slimmer and more refined line.


As in this case, when functional issues and aesthetic improvement are addressed at the same time,
you can achieve both more comfortable breathing and a satisfying change in appearance.
