Hello, this is Apgujeong Baba Plastic Surgery.
Many people think that only the bridge or the tip of the nose is important in rhinoplasty, but in fact, the columella is a key point that determines the overall nose line and the impression of the face.
Today, Baba Plastic Surgery will explain the importance of the columella and summarize our design and surgical know-how.

When the columella is recessed / When the columella is lowered
- When the columella is recessed (common in East Asians)
In reality, cases where the columella has significantly dropped are rare, and a recessed shape is more common.

Front view: the nostrils may appear more prominent

Side view: an impression that the alar tissue is sagging
If you want a sharper nose line, we recommend combining columella correction with rhinoplasty.

- Why a pretty nose bridge can still look like it had surgery = a broken columella line
Even if the bridge is neat, if the line from the tip to the columella looks broken, the nose can appear unnatural.

Solution: reinforcing the columella volume with cartilage grafts to bring out the curve improves it into a more natural nose line.

- When the columella looks long (hooked nose / continuity)
If the columella appears long, the nose can look like a hooked nose and may give the face a more aged impression.
Correction: tie the columella to the septal cartilage to reduce the length
If the septum itself is long and pushed downward: part of it is removed and then corrected by pulling it inward.

- When the alar tissue is actually sagging = consider alar lift
If it is not a visual illusion but actual sagging, it is lifted with an alar lift.
However, because the before-and-after change may not be dramatic and the maintenance period may not be long, it should be chosen carefully only when truly necessary.

- Summary of surgical methods for ‘lowering’ the columella
When raising the nasal tip, rib cartilage or septal cartilage is used, and the direction is pushed toward the columella to induce columella descent.
If there is not enough, additional remaining cartilage is grafted to extend the columella length.

- Apgujeong Baba Plastic Surgery’s columella surgery know-how
First, we do not use donated rib cartilage alone X → we use septal cartilage and ear cartilage together to strengthen the stability of the support structure
Second, minimizing scars: harvesting is done from less visible areas such as behind the ear
Third, safety first: the amount harvested is determined with the patient’s condition and safety as the top priority
[Material selection guide]
If possible, autologous rib cartilage is recommended first
If that feels burdensome, we suggest an alternative combination of donated rib cartilage + septal cartilage + (if needed) ear cartilage
Columella correction is recommended for people like this
When the nostrils look prominent from the front
When the sagging of the alar tissue is bothersome in side view
When the bridge is nice but you are told the nose looks like it had surgery
When the hooked-nose feeling or a nose that looks long makes the face appear more aged
✅Pre-procedure checklist
The plan differs depending on each person’s septal condition, skin thickness, and amount of cartilage available.
To prevent overcorrection, it is important to design the line while considering the individual’s structure.

Frequently Asked Questions (FAQ)
Q1. Can columella correction alone change the impression?
A. It depends on the case, but when the columella line is enhanced, the continuity between the nasal tip and the columella improves, which can help the nose line look more natural.
Q2. Won’t the scar be noticeable?
A. Cartilage harvesting usually uses less visible areas such as behind the ear. The approach is explained differently depending on the individual condition.
Q3. Autologous rib cartilage vs donated rib cartilage, which is better?
A. Autologous rib cartilage is more stable, but if that feels burdensome, sufficient support can still be secured with a combined graft of donated rib cartilage + septal cartilage + ear cartilage.
The completion of rhinoplasty is not determined by bridge height alone.
A small but important columella correction changes facial proportions and the overall impression.
Apgujeong Baba Plastic Surgery analyzes each individual’s facial structure and proposes customized rhinoplasty and columella correction.
