Real case of improving a bulbous nose complex with rhinoplasty without implants
Among those considering rhinoplasty, many people in particular feel stressed because of a bulbous nose, thick nose, or hooked nose.
If the nasal tip looks round and wide, the face can appear heavy, and if the nostrils spread outward, the front view can make the nose look larger.
In a recent YouTube video released by BABA Plastic Surgery,
you can see a real male model case in which concerns about a bulbous nose were improved through rhinoplasty without implants.
Today, I will explain in detail what surgical points made the change look natural in this case.

Model's concern points: hooked nose + bulbous nose + wide nostrils
Before surgery, this model had the following concerns during consultation.
Nasal line with a hump
A bulbous nose that looked round and wide from the front
Spread nasal wings and large nostrils
A blunt overall impression, like a bulbous nose

In such cases, the key is not simply to raise the nose,
but to focus on structural correction and design.

The glabella was already on the high side → proceeded without an implant
Because the model's glabella area was already relatively high,
instead of inserting a silicone implant and making it too high,
we proceeded in the direction of shaping a natural line with rhinoplasty without implants.
Rhinoplasty without implants preserves the patient's own nasal structure
and corrects only the necessary parts,
so it has the advantage of looking less artificial and being more natural and less burdensome, even in male rhinoplasty.

Hooked nose correction: not just shaving, but refining the line as well
One of the important points in this surgery was
removing the hump.
Shaving down the hump softens the line,
but at the same time it can create one problem.
✔ While shaving the hump, the bridge of the nose can look thicker.
So at BABA Plastic Surgery, it wasn't just about shaving it down;

we also performed lateral osteotomy,
which folds the side walls of the nose inward.
Through this process, the bridge of the nose was refined again and became slimmer,
making the straight line even more distinct.

Nasal tip design: naturally heightening it to match the straight line
After correcting the hump,
the height of the nasal tip also needs to be adjusted to match the overall line.
For the model, the design was adjusted so that the nasal tip was naturally lifted
in line with the straight line created after removing the hump.
The key was not just raising the bridge, but creating a line that connects evenly all the way to the tip.

Bulbous nose correction: making the round nasal tip slimmer from the front
When the nose looks round and full from the front,
it corresponds to a typical bulbous nose type.
In such cases, the nasal tip cartilage structure is refined
and unnecessary tissue volume is reduced,
so that the nasal tip looks slimmer through bulbous nose correction.
After bulbous nose correction, even if the bridge is not very high,
the front impression can become much sharper.

Alar reduction: both medial and lateral excision performed
The model had a lot of spread in the alar tissue,
and the nostrils were also rather large.
In particular, because the nostrils spread outward and lifted upward significantly,
both medial excision and lateral excision were performed together.

With this approach,
Improvement in flaring of the nostrils
Reduction in nostril size
A narrower nose width from the front
can all be achieved at the same time.

Columella correction: improving the nasolabial angle + lifting a drooping columella
The last important detail is
columella correction, the structure below the nasal tip.
The model had a deeply recessed nasolabial angle between the nostrils,
and the columella was relatively drooped.

Therefore, during surgery,
✔ The nasolabial angle was brought downward
✔ The drooping columella was lifted and corrected with the design
creating a clearer and cleaner impression at the nasal tip.

A natural transformation completed with straight-line rhinoplasty without implants
This case improved a bulbous nose complex naturally without implants through
hump removal
lateral osteotomy to refine the bridge
bulbous nose correction
alar reduction (medial + lateral)
and detailed columella correction
If you want a straight-line male rhinoplasty that is not excessive and suits your face,
rhinoplasty without implants can also be a good option.
