Sometimes, younger patients come in after having eye surgery and then, not long after, return to have anti-aging eye surgery for their parents.
A representative example is upper blepharoplasty.
The skin around the eyes moves a lot and is relatively thin, so it is one of the areas where aging appears early.
In particular, when the skin tissue of the upper eyelid sags unattractively, it not only creates a heavy-looking impression overall, but can also cause functional problems with the eyes.
Sometimes, patients in their 70s or older visit us saying that their field of vision is so obstructed that they often stumble and fall.
Because this can lead to a serious accident if it is viewed only as a simple skin-sagging issue, prompt correction is often needed.
However, for upper blepharoplasty to produce the most ideal result, both functional and aesthetic aspects need to be considered.
Since the surgery is performed on a highly visible area, satisfaction with the outcome must also be taken into account, not just the treatment purpose.
If too much tissue is removed just because the skin is very saggy, it can actually cause side effects, so there are many points to consider.
So, how should upper blepharoplasty be performed to achieve a desirable result?

What is the key point of upper blepharoplasty?
Upper blepharoplasty involves making an incision at an appropriate position on the upper eyelid, removing the sagging tissue, and creating a double eyelid.
If you already have a double eyelid and are not particularly dissatisfied with it, the tissue is removed through an incision in that area.
If you do not have a visible double eyelid, such as with monolids or an inner double eyelid, we design the ideal line after sufficient consultation with the patient.
But there is one thing to be careful about here as well.
If the skin is severely sagging and excessive removal is performed, the double eyelid line can become too thick and lead to an unnatural result.
Therefore, it is important to determine whether the degree of eyelid sagging is something that upper blepharoplasty can adequately address and to perform surgery accordingly.

Upper blepharoplasty is usually recommended for the following people:

When the double eyelid line has sagged overall due to drooping eyelids

When the eyelids are severely drooped and block the field of vision

When wrinkles around the eyes are so pronounced that you look older than you actually are
However, even in these cases, if an unnatural result is predicted because of a thick double eyelid line, other surgical methods can be considered.

Can buried-suture double eyelid surgery also be done with upper blepharoplasty?
Because tissue is usually removed, many people think upper blepharoplasty can only be performed using the incision method for double eyelids.
However, if a thick double eyelid is expected to protrude awkwardly like a sausage, surgery may also be performed in a way that creates a buried-suture double eyelid.
This involves partially removing tissue in a direction that is not clearly exposed as the double eyelid line and then creating the double eyelid using the buried-suture method.
However, if buried sutures are used when the eyelids are drooping, the upper part of the double eyelid may droop again and cover the original line, and over time there may be a higher chance that it loosens.
To prevent this, at our clinic we sufficiently remove the sagging skin through excision surgery to help produce a natural result.

Upper blepharoplasty requires more considerations than general eye surgery.
First of all, it deals with sagging skin, and not only the skin but also issues involving fat or muscle may be involved, so the surgical conditions themselves are much more demanding.
