As life expectancy rises among people in Korea, more and more people are continuing their social lives for a long time, making the term “retirement age” feel almost outdated.
Because of this, these days there are many people who want to live each day looking younger and more beautiful, even if it is only for a day.
At our clinic as well, many patients visit us saying things like, “The skin around my eyes has sagged so much that it’s bothering me,” or “I’ve become withdrawn and my self-esteem has dropped a lot.”
Today, we’d like to look at two procedures that may be recommended for such people: upper blepharoplasty and lower blepharoplasty.
But wait a moment.
Before getting into the main topic, there is one thing I’d like to say.
In the case of eyelid surgery for middle-aged patients, it is much more complicated than the eyelid surgery applied to people in their 20s and 30s.
That is because the skin elasticity has significantly decreased, and the eyes may also have functional issues such as irritation around the eye area.
Also, each patient has different lifestyle habits, so even if they are the same age, the severity of symptoms can vary.
Therefore, you should keep in mind that ideal results can only be achieved through a design and surgical method tailored to each individual.

About upper blepharoplasty and lower blepharoplasty
- Upper blepharoplasty
The eyelids are a part of the face where aging appears quickly because they move frequently, such as when blinking, and because the skin is thin.
Also, since the eyeball sits beneath them, sagging upper eyelids can cause various problems, including obstruction of vision.
At this point, applying upper blepharoplasty can create eyes that look firmer and smoother.

Upper blepharoplasty refers to a procedure in which an incision is made along the line where the double eyelid crease will be formed, and then some of the sagging tissue is removed before suturing.
If there is already a double eyelid crease and the patient is still satisfied with it, the incision can be made along that line.
If you do not have a double eyelid crease, you should of course expect one to be created after the surgery. ^^
Because sagging skin and fat are removed while the double eyelid is also formed at an appropriate size, it can create a much younger-looking and firmer eye shape.
However, there is one thing to be careful about.
Only the necessary amount should be removed; if too much is removed, there is a risk that a thick double eyelid crease may form.
Therefore, after sufficient consultation, the necessary amount of removal should be determined and applied. Even so, if there is a possibility that the double eyelid line may look awkward or become too dark, the sagging eyelid should be addressed with a forehead lift or brow lift, while considering buried-suture double eyelid surgery.
- Lower blepharoplasty
Lower blepharoplasty is recommended for people who are worried about sagging and protruding skin beneath the eyes.
It is a procedure that removes or repositions the fat under the eyes to create flatter skin and improve wrinkles and elasticity.


If the decrease in elasticity is not severe, a transconjunctival lower blepharoplasty can be performed, in which a tiny incision is made inside the lower eyelid and the fat is repositioned.
Of course, there is no visible scar on the surface, and the surgery time can also be shortened significantly.
If the skin sagging is severe or wrinkles are somewhat noticeable, a transcutaneous lower blepharoplasty is performed.
Transcutaneous lower blepharoplasty means making an incision just below the lower eyelid lash line and performing lifting along with fat repositioning.
The skin and muscle are pulled upward and fixed, then the procedure is completed by suturing with fine thread so that the incision is hidden beneath the lower eyelashes.
However, there is also something to be careful about when performing lower blepharoplasty.
When the under-eye area protrudes too much, people usually think, “Then just remove all of this fat.”
But it is important to keep in mind that as aging progresses, not only collagen but also fat decreases.
If too much fat is removed simply because the under-eye area is currently very bulging, the under-eye area may become noticeably sunken over time, and a grafting procedure may be needed again.
The key to lower blepharoplasty is that, along with fat repositioning, lifting that firmly supports the fat must also be performed.
In this way, after surgery, it is possible to create eyes that look firm and healthy without under-eye hollowness.

Today we looked at upper blepharoplasty and lower blepharoplasty.
You may have thought these procedures were only about simple excision and fat repositioning, but they require more care than you might expect, don’t they?
Next time, I’ll talk about why the lateral canthoplasty that many young women undergo these days can actually become more necessary as people age, and why it is recommended to have it done together with lower blepharoplasty.
