Today’s main topic is <mid-aged eye surgery>.
As life expectancy rises and more people remain active in society even after retirement, <mid-aged eye surgery> has become one choice for middle-aged people that helps make time feel like it is passing a little more slowly.
As aging progresses, the eyelids begin to sag. In particular, for people with severe upper eyelid drooping, this can also cause functional problems with the eye itself, so many decide to undergo surgery urgently.
However, removing stretched tissue through upper eyelid surgery does not solve everything.
Because the eyes are the first area people notice when meeting someone, both functional and aesthetic concerns must be considered together.
On the other hand, even if surgery is performed for cosmetic purposes, doing too much can cause side effects such as being unable to fully close the eyes, so functional aspects must definitely be preserved.
To do that, it is not enough to simply cut and pull the skin. The root cause must be improved from the deeper layers. Today, we will explain that method. :)

Mid-aged eye surgery should not focus only on the issues that are visible right away. Looking at the eyes of people in their 50s and older, they often seem somehow heavy or constricted.
As the eyelids sag, the outer edges of the eyes curve downward. The outer corners of the eyes droop severely, almost like a teardrop laid on its side, and as a result, the person looks tired and gives off a negative impression.
What about the lower eyelids?
As skin elasticity declines, the fat surrounding the eyeball can protrude outward and become more prominent, while the tear trough becomes relatively hollow, creating deep wrinkles.
These changes do not appear in just one area; they occur in a complex way across both the upper and lower eyelids, so it is important to look at the overall structure when performing surgery.
If you simply remove tissue from the area where the eyelid is sagging right now, it may instead lead to an awkward and unnatural result. Also, because attention is placed only on the skin rather than the muscle, there is a real chance that it will sag again quickly as time passes.
In general, mid-aged eye surgery involves many more factors to consider than the procedures performed in your 20s and 30s, and the correction range is also relatively broader.
Sometimes, surgery is carried out with excessive focus only on trimming and removing fat, but in such cases, people often regret it so much that they later consider revision surgery.
When operating on the eyelids, please remember that it is important not only to address that specific area, but also to consider functional issues after surgery and the overall harmony of the upper and lower eyelids, so that fundamental changes can be made.

- Upper eyelid surgery
Upper eyelid surgery refers to an operation that corrects a sagging eyelid back to its previous elastic and healthy state.
As mentioned briefly earlier, the important point here is to be careful not to remove too much tissue.
Even if there is significant sagging, removing too much tissue can inadvertently create a very thick double eyelid.
In particular, if the double eyelid crease is positioned incorrectly, it may look like a sausage-like double eyelid. And if there is true ptosis (a condition in which the muscle that opens the eye becomes loose), the eyes can look even sleepier.
So how should surgery be performed in such cases? When people hear “upper eyelid surgery,” most think only of the “incision” method.
Perhaps that is because tissue removal is considered absolutely necessary.
That is why, at our clinic, when the eyelid is severely sagging and we determine that tissue removal beyond a certain standard is needed, we make a separate small incision to remove tissue, but finish the double eyelid using the “buried suture” method.
Also, if we determine that true ptosis is present and the eyes are likely to still look sleepy even after upper eyelid surgery, we perform ptosis correction together to strengthen the ability to open the eyes.
As such, it is not simply a matter of cutting because the eyelid is sagging; it is important to approach the case while considering the overall structure and function.

- Lower eyelid surgery
Lower eyelid surgery is divided into transconjunctival surgery and transcutaneous surgery. The reason for this division is that the surgical methods themselves are completely different.
Transconjunctival surgery is a procedure in which an incision is made on the red inner tissue that appears when the eyelid is turned inside out, and the internally bulging fat is repositioned.
This method does not greatly help with under-eye wrinkles or elasticity, so we recommend it for people who do not have wrinkle or elasticity problems.
On the other hand, transcutaneous surgery involves making an incision in the lower skin where the eyelashes grow, so compared with transconjunctival surgery, there is quite a bit more that can be done.
Not only can the fat be repositioned, but the skin and muscle can also be pulled together for a lifting effect, so it can be very helpful for improving deep wrinkles and elasticity.
Among middle-aged patients, those who are relatively younger tend to undergo transconjunctival surgery (with no visible scar), while for those in their 60s and older, transcutaneous surgery is generally performed.
