When people say “middle-aged eye surgery,” the procedure that usually comes to mind is “upper blepharoplasty.” Simply put, upper blepharoplasty can be described as “double eyelid surgery for middle age.”
After making an incision along the double eyelid line, part of the aged, weakened skin tissue is removed through that area and then sutured again. As a result, when the eyes are open, the excess sagging skin is reduced, a double eyelid is created, and the face can appear more youthful.
However, during surgical consultations, there are some difficult cases as well. These are people whose skin sagging is so severe that their eyes have become very small.
Nowadays, because many people take care of themselves, this is rarely seen until their 50s. But once people reach their 60s or 70s and older, it becomes relatively common.
Even if the same upper blepharoplasty technique is used, much more skin must be removed than in upper blepharoplasty performed in the 50s. If done incorrectly, the double eyelids can look too thick and unnatural.
When the eyes are closed, the trace may remain too pronounced, or when the eyes are open, the impression may become unnatural, like a sausage-like double eyelid.
So what kind of upper blepharoplasty is best for these patients?

Middle-aged eye surgery = upper blepharoplasty? Something feels off.
Even eye surgery has many different types, so there is no way that middle-aged eye surgery would only mean one procedure: upper blepharoplasty.
If, because there is skin sagging, you try to remove a lot of skin only through upper blepharoplasty, the more skin is removed, the harsher the impression becomes. In addition, the distance between the eyes and eyebrows becomes that much narrower, increasing the likelihood of a sausage-like double eyelid.
That is why upper blepharoplasty is highly satisfying for most patients when performed on people in their 40s to 50s whose eyelid sagging is not that severe and whose eyes and eyebrows are moderately far apart.

Upper blepharoplasty: how can you avoid a sausage-like double eyelid?
When people hear upper blepharoplasty, they often think it only involves an incision method.
This is not different from what was mentioned above. Because it is double eyelid surgery for middle age, people tend to think that an incision must be made along the double eyelid line to remove tissue.
If this is done when skin sagging is too severe, the face can look harsh after surgery. So from another perspective, you need to ask, “How can we create a natural double eyelid?”
That is why our clinic recommends sub-brow lift surgery and buried suture double eyelid surgery.
Of course, sub-brow lift surgery cannot be said to be suitable for everyone. However, in cases where the distance between the eyes and eyebrows is moderately spaced but the eyelid sagging is too severe and a large amount of skin would need to be removed, performing surgery this way allows sufficient removal while creating a natural double eyelid with the buried suture method, which can produce a softer and more stable impression.
Sub-brow lift surgery is a procedure in which a small incision is made as much as possible in a hidden area based on the lower edge of the eyebrow, unnecessary skin is removed, and sagging tissue is lifted.
After lifting is performed appropriately through sub-brow lift surgery, a double eyelid is created with the buried suture method according to the line you want.
Of course, if you already have a double eyelid, or if you have monolids and do not want to create a double eyelid, it is fine to undergo only sub-brow lift surgery. This can also be decided after sufficient consultation with our doctors.
Middle-aged eye surgery must look natural.

In addition, because middle-aged eye surgery must look natural, it should be approached by considering the degree of eyeball protrusion, the degree of orbital bone development, each person’s facial features, and the relationship with surrounding skin sagging.
