Everyone. When you hear middle-aged eye surgery, what procedure comes to mind first? For most people—more than 90%—the first procedures mentioned are probably upper blepharoplasty and lower blepharoplasty.
But did you know that there are also several different methods for upper blepharoplasty and lower blepharoplasty?
People each have different eyelid shapes. For example, even if someone needs upper blepharoplasty because the upper eyelid has sagged, their detailed needs can still vary from person to person.
Some may say, “I only want to lift the sagging eyelid,” while others may say, “I want to remove the sagging skin while creating a double eyelid.”
Among people who already have double eyelids, some still have well-maintained folds that have not loosened, while others have seen their original double eyelids loosen with age.

The same is true for lower blepharoplasty.
Some people want to improve the bulging under-eye area while also reducing wrinkles at the same time, while others still have firm under-eyes without wrinkles but want to flatten only the slightly protruding fat and the relatively sunken orbital bone.
As you can see, even though the procedures are called upper blepharoplasty and lower blepharoplasty, the surgical method can differ depending on the individual. That is why it is important to choose a board-certified plastic surgeon who listens carefully to each patient and has sufficient clinical experience with middle-aged eye surgery.

Upper Blepharoplasty and Suitable Candidates
Upper blepharoplasty refers to a procedure that improves sagging eyelids and wrinkles caused by aging, and creates a double eyelid that suits the individual well, restoring a more youthful-looking eye area.
Because the surgery is performed on skin that has lost elasticity, the most important point is to appropriately remove stretched skin, the orbicularis oculi muscle, and fat so that the overall face does not look awkward and the result appears as natural as possible.
Many people may be wondering about the right time for surgery.
Whenever possible, surgery is best performed early, when the eyelids first begin to sag, in order to help maintain eyelid elasticity. This leads to faster recovery after surgery and better results.
Some middle-aged men who continue to work in society, or women who do not want their appearance to change because they feel burdened by cosmetic surgery, may also be concerned.
In such cases, there is also a method of lifting the eyelid by removing only the sagging skin without creating a double eyelid.

Lower Blepharoplasty and Suitable Candidates
The upper eye area has a more complex structure, including the eyebrows, so improving only sagging may be enough. However, the under-eye area sits far below the orbital bone surrounding the eyeball, so along with skin sagging, various changes can appear.
As the skin becomes thinner and the muscles around the eyes weaken, wrinkles form, and the septum that supports the orbital fat loosens, causing the under-eye area to bulge.
When these changes occur with age, they can create a stern or grumpy-looking expression and become a negative image in social settings.
Lower blepharoplasty is a procedure that removes under-eye wrinkles and also addresses bulging fat pockets by repositioning them around the orbital bone.
However, as with upper blepharoplasty, the degree of aging differs from person to person, so the procedure must be carried out accordingly.

In severe cases where the eyelids are significantly sagging and vision is not properly 확보되지 않는, in addition to the upper blepharoplasty described above, other procedures may be performed together.
