One of the things foreigners are most surprised by when they visit Korea is that they “can’t tell people’s ages.”
It’s common to see people who look like they are in their 30s or 40s but are actually in their 50s, and people who look like they are in their 50s but are actually in their 70s.
When working at a plastic surgery clinic, we often end up staring intensely at someone’s face every day, and even we are surprised more than once or twice.
The reason I mention this is that when people hear “middle-aged eye surgery,” many approach it with an understanding such as, “Upper blepharoplasty is a surgery that removes sagging eyelid skin and creates a double eyelid,” and “Lower blepharoplasty is a surgery that removes under-eye fat and improves wrinkles.”
As mentioned earlier, everyone ages differently, and symptoms appear differently as well.
Even if someone is middle-aged, performing a uniform procedure like the one above can, if anything, lead to burdensome and unnatural results, so caution is needed—again and again.
So then, what is the correct way to approach middle-aged eye surgery?
Today, at Reborn Plastic Surgery, we will explain middle-aged eye surgery carefully and clearly, from start to finish.
◆ Various Methods of Upper and Lower Blepharoplasty
The “upper blepharoplasty” and “lower blepharoplasty” that we commonly know have the following purposes.
· Upper blepharoplasty = Removes sagging skin tissue from the upper eyelid and creates a double eyelid to restore a younger-looking eye shape.
· Lower blepharoplasty = Removes bulging under-eye fat and lifts skin that has lost elasticity to create a more lively eye shape.
However, these are not the only surgical methods available.
That is because each person may want a different eye shape, and if the structural form of the eyes they were born with does not suit that, the direction of surgery itself must be changed.

For Upper Blepharoplasty
For example, suppose someone wants upper blepharoplasty because their upper eyelids are drooping, but they already have a clear double eyelid line?
What if that double eyelid is simply being covered by the sagging eyelid skin?
Creating a new double eyelid line may feel burdensome and may even lead to an unnatural result.
In that case, a sub-brow lift, which only removes the sagging skin without creating a double eyelid, becomes a better choice.
Also, what if making a double eyelid is acceptable, but there is so much skin that needs to be removed that after surgery the result could become a “sausage-like” double eyelid, or the eyelid crease may look excessively thick?
Many people think upper blepharoplasty always means making a double eyelid through an incision, but that is not necessarily true.
If there is concern that the double eyelid will become too pronounced, a very small incision can be made in the upper area to lift the eyelid, and then the double eyelid can be recreated in the form of a non-incisional double eyelid.

For Lower Blepharoplasty
Lower blepharoplasty is not much different.
There are two types of lower blepharoplasty.
They are “transcutaneous lower blepharoplasty” and “transconjunctival lower blepharoplasty” (through the red conjunctiva on the inside of the eyelid).
Even if two people are both 50-year-old middle-aged adults, some may still have elasticity under the eyes, while others may have severe loss of elasticity and fine wrinkles.
Of course, the same surgical method should not be used in both cases.
If elasticity remains under the eyes, an incision can be made on the red conjunctiva inside the eyelid to reposition only the protruding fat into the hollow area and smooth it out.
Because the incision is made in the conjunctiva, there is no chance that the incision site will be visible from the outside, and the surgery is completed quickly.
However, if elasticity is severely reduced and wrinkles are present, the skin area hidden by the lower eyelashes must be incised so that the fat can be repositioned while also performing an elasticity lift.
If transconjunctival lower blepharoplasty is performed on such a person, the loss of elasticity and wrinkles will remain as they are, and satisfaction will drop significantly.
As such, you must remember that the surgical method should be applied differently depending on the patient’s eyelids, fat, muscles, structural features of the eyes, and needs, and choose a plastic surgery clinic accordingly to increase satisfaction.

◆ The Key to Middle-Aged Eye Surgery Is a “Natural Result”
Middle-aged eye surgery is absolutely not a procedure in which you should overdo things.
If you get greedy and remove too much skin and fat, the eyes may become overly wide-open, and over time this can lead to a deeper sunken look under the eyes.
You need to consider harmony with the condition of the skin in other areas, because eyes that look too intense by themselves can feel burdensome.
The eyes play a major role in shaping the impression of the face.
