Even for the same upper blepharoplasty, the detailed procedure may differ depending on the patient’s needs and the current condition of the eyes.
Today, I will introduce the various types of eyelid surgery commonly performed by middle-aged patients.
Upper Blepharoplasty
Upper blepharoplasty is a surgery that improves drooping eyelids by removing excess skin from the sagging eyelids and tightening the weakened muscle.
- For those whose upper eyelid skin sagging is severe and makes them look older
- When sagging has disturbed the double eyelid line
- In situations where drooping eyelids are obstructing vision
This is a procedure that can be considered in such cases.
After surgery, it may help improve problems with restricted vision, and it can also make the eyes appear clearer and more lively.
In addition, it can help improve other functional issues as well, such as eyelashes poking the eyes and irritation around the eye area.

Brow Lift Surgery
A brow lift is recommended when skin sagging is so severe that too much tissue would need to be removed with upper blepharoplasty alone.
A brow lift is a surgery that lifts the covered area and sagging tissue based on the lower edge of the eyebrow.
After that, the double eyelid line is refined using the buried suture method to create a softer and more stable impression.
However, it is difficult to say that this surgery is suitable for everyone.
A certain distance between the eyebrow and the eye must be secured for the surgery to be possible.
Otherwise, the gap may become too narrow, which can make the overall impression look awkward.

Ptosis Correction
Ptosis correction is surgery to correct blepharoptosis.
Blepharoptosis refers to a condition in which the eyes droop because the muscles that open the eyes are weak.
As aging causes the skin to sag, the muscles beneath the skin can loosen, reducing the strength needed to lift the eyelids.
Therefore, if the eyes are half-closed or feel limp and droopy, or if a person excessively uses the forehead or eyebrows to open the eyes, this is a method to correct it.
Ptosis correction is not something that can be done in every case; surgery is determined when the eyelid covers more than 2 mm of the black part of the eye.
The procedure is performed by strengthening the levator muscle of the eyelid.

Lower Blepharoplasty
Lower blepharoplasty is a surgery that improves excessively protruding under-eye area.
As people age, skin elasticity decreases, causing the skin to become thinner and sag.
When the orbital fat around the eyes collects under the eyes and bulges outward, dark circles become more noticeable and the overall impression can look darker and older.
At this time, lower blepharoplasty can redistribute the protruding fat and improve sagging skin, creating a livelier and younger-looking impression.

Under-Eye Fat Repositioning
If the sagging or wrinkles under the eyes are not severe and you only want to improve the bulging area, we recommend under-eye fat repositioning.
Under-eye fat repositioning is a method in which only the fat is repositioned by making an incision in the conjunctiva on the inside of the eyelid.
It can be performed on people whose under-eye elasticity is still present and who do not have severe wrinkles or sagging.
Because the skin hidden by the eyelashes is incised, the incision site is not exposed after the procedure, and the surgery also finishes quickly.
However, if elasticity has significantly decreased and wrinkles are deep, lower blepharoplasty involving a skin incision may be more helpful than this.

As such, the direction and method of surgery are applied differently depending on the patient’s eyelids, fat, muscles, structural features of the eyes, and degree of aging.
Everyone has different eye shapes, degrees of skin sagging, and fat positions.
