※ 3 months after the second eyelid fat grafting procedure
※ The results of all procedures and surgeries may vary depending on an individual’s skin condition, constitution, and other factors. There is also a possibility of side effects such as bleeding, swelling, bruising, inflammation, infection, pain, asymmetry, and nerve damage. A careful decision should be made after sufficient consultation.
Some people are concerned because the area above their eyes looks hollow whenever they look in the mirror, or because their eyes appear more tired than before.
Hollowing of the eyelids is often more than just a change in appearance.
This is because it can also be accompanied by easily fatigued eyes or changes in the appearance of the double-eyelid crease.
For this reason, more people have recently become interested in eyelid fat grafting as one way to improve a hollow-looking eye area.
However, the area around the eyes has very thin and delicate skin, so adding more fat does not necessarily produce a better result.
Rather, excessive volume may make the eyes look heavy or lead to an unnatural appearance.
Why do the eyelids gradually become hollow?
Eyelid hollowing can occur for a variety of reasons.
Common causes
✔ Loss of fat due to aging
✔ Reduced elasticity of the tissues around the eyes
✔ Decreased volume in the upper eyelid area
✔ Thinning skin
✔ Congenital bone structure
✔ Excessive dieting
✔ Changes following previous eye surgery
In particular, as people age, the fat above the eyes decreases, making it easier for deep shadows to form above the eyelids.
This can make the eyes look more tired and sharp, creating an impression that makes a person appear older than their actual age.
The existing double-eyelid crease may also fold into multiple layers, or the crease may appear excessively deep.

Could hollow eyelids also be related to eye fatigue?
Eyelid fat does more than simply provide volume; it also helps the eyelids move smoothly when opening and closing the eyes.
In simple terms, it functions somewhat like a cushion.
However, if the fat above the eyes decreases excessively, the eyes may feel stiff when opening and closing, become easily heavy, or feel increasingly fatigued as the afternoon progresses.
In other words, eyelid hollowing is not merely an appearance-related issue; it may also be connected to eye movement.
How is eyelid fat grafting performed?
Eyelid fat grafting uses the patient’s own fat to restore volume in hollow areas.
A small amount of fat is usually harvested from areas such as the abdomen or thighs. The necessary fat is then purified and delicately grafted into the eyelid area.
Because the skin around the eyes is very thin and the area moves frequently, a particularly precise approach is required.
- Which areas are hollow
- How much volume is needed
- How the result will harmonize with the eyes
These factors must be carefully considered to achieve a natural-looking result.
One of the most important aspects of eyelid fat grafting is achieving “natural-looking volume.”
If too much fat is added simply because the area above the eyes is hollow:
✔ The eyes may look heavy
✔ It may feel like swelling
✔ The overall impression may look dull
✔ It may lead to an unnatural appearance around the eyes
The eyelids are an area where even a very small change can significantly alter the overall impression of the face.
Therefore, it is important to adjust the volume appropriately so that it is neither excessive nor insufficient.
You may consider eyelid fat grafting in the following cases.
| ✔ The upper eyelid area looks deeply hollow✔ The double-eyelid crease appears excessively deep✔ Multiple crease lines have formed✔ You are concerned that your eyes look tired and aged✔ The shadow above the eyes appears dark✔ You want a natural and gentle appearance |
In particular, when reduced volume above the eyes has made the eyes look sharper, fat grafting may help create a gentler appearance.
Many people are also concerned about the lower-eyelid fat area.
Some people have hollowing above the eyes while, conversely, the lower-eyelid fat protrudes and appears puffy.
In such cases, one possible approach is to use the lower-eyelid fat to supplement volume above the eyes.
In other words, this approach considers both “improving lower-eyelid puffiness” and “supplementing hollowing above the eyes.”
Therefore, rather than examining only one area, it is important to assess the overall balance of the areas above and below the eyes.

Why is eyelid fat grafting challenging?
The eyelids are an especially sensitive area of the face.
Because the skin is thin and the area moves frequently, meticulous technique is required during fat grafting.
Why is it challenging?
- The skin is very thin
Even a slight excess can result in an uneven texture or unnatural volume.
- Balance between the two sides must be considered
Human eyes are not naturally perfectly identical.
Differences in the strength of eyelid opening and bone structure must also be considered to achieve a more natural result.
- Overall facial harmony is important
Simply filling the area above the eyes does not necessarily create a natural appearance.
The volume of the midface, the position of the eyebrows, and the overall impression of the eyes must also be considered.
Fat grafting alone may be sufficient in some cases
When eyelid hollowing is not severe and there is little skin sagging, natural improvement may be possible with fat grafting alone.
Conversely, if skin sagging has also developed, it may be more appropriate to consider another approach, such as upper blepharoplasty, along with fat grafting.
The important point is not to decide on a particular surgery unconditionally, but to thoroughly analyze the current condition of the eyes.
Extra caution is needed if you have a history of revision surgery or previous eye surgery.
This is because the condition of the tissues may have changed if you have previously undergone double-eyelid surgery or a procedure to improve the appearance of the eyes.

Most people considering eyelid fat grafting want similar things.
- I hope it does not look too noticeable
- I want to look naturally more refreshed
- I want to achieve a gentler appearance
However, these results cannot be achieved simply by adding a large amount of fat.
The current condition of the eyes and the overall appearance of the face must be fully considered, with a delicate approach using only the amount needed to pursue a more natural result.
Eyelid hollowing is one of the relatively common changes that can occur during the aging process.
However, if you are increasingly concerned because your eyes continue to feel tired or your appearance seems excessively fatigued, please receive sufficient consultation about the approach suited to your current condition.
