Many people are concerned about hollowing in their eyelids due to aging.
When the eyelids begin to look hollow, it does not simply create an older-looking appearance.
The double-eyelid line may develop multiple folds, and the eyes may also become tired more easily.
In such cases, fat grafting above the eyes may be considered. However, one important point is that the eyelids are an area with very thin skin and frequent movement, so simply adding a large amount of fat does not necessarily produce a good result.

Why does hollowing above the eyes occur?
Hollowing above the eyes can occur for various reasons.
The most common reason is aging.
As people age, skin elasticity decreases, and the fat above the eyes gradually diminishes as well.
As a result, the eyelids may appear sunken and develop shadows.
There is one aspect that many people overlook: the fat above the eyes is not simply tissue that creates volume.
It also helps the eyelids move smoothly when opening and closing the eyes, so a decrease in fat may cause the eyes to become tired more easily.
In fact, people with hollow eyelids may feel that
✅ their eyes feel heavy
✅ they become easily tired in the afternoon
✅ their ability to open their eyes is not what it used to be
There are also many cases in which changes to the double-eyelid line occur along with hollowing above the eyes.
The original line may fold into multiple layers, the double eyelid may appear excessively thick, or the line may look less defined.
When deep shadows also develop, they can create an older-looking appearance.
Of course, not all hollowing of the eyelids is caused only by aging.
Fat may be lost after a rapid decrease in body weight, and some people naturally have little fat in the upper eyelid area.
In other cases, hollowing may become more noticeable because of insufficient volume after previous eye surgery or eyelid correction.
Therefore, it is important to examine the current eye structure and identify the cause.

Why is the fat graft survival rate important for fat grafting above the eyes?
Fat grafting above the eyes is a method of filling hollow areas using the patient's own fat.
A small amount of fat is usually harvested from the abdomen or thighs, after which only healthy fat is purified and injected into the eyelids with great precision.
The key factor in fat grafting is the “fat graft survival rate.”
This refers to how well the transplanted fat survives in the body.
In other words, the fat must connect well with blood vessels and settle into place stably for natural-looking volume to be maintained in practice.
If the graft does not survive sufficiently, the area may initially appear to have volume, but much of the fat may be lost over time.
The area may then look hollow again, or differences between the two sides may develop.
Conversely, adding too much fat simply to increase the survival rate is not necessarily a good approach.
Because the eyelids have very thin skin, excessive injection may result in
✅ a thick, heavy-looking eye area
✅ unnatural volume
✅ an uneven texture
Therefore, with fat grafting above the eyes, it is very important to inject healthy fat delicately, in appropriate layers, and in a naturally distributed manner.
Because the eyelids are an area with frequent movement, a much more precise approach is needed than with general facial fat grafting.
In addition, skin thickness, the strength of the muscles that open the eyes, and the amount of fat all vary from person to person, so it is important to create a plan suited to the current condition.
If there is little skin laxity and the cause is simply insufficient volume, natural improvement may be expected with fat grafting alone.
Conversely, if the skin is significantly loose, an upper blepharoplasty may also need to be considered.
In other words, the important thing is not to apply one method unconditionally, but to take an approach suited to the current condition of the eyes.

Hollow eyelids do not merely cause an external change.
Many people also feel stressed as their eyes become tired more easily and their overall appearance changes.
However, because the eyelids are an extremely thin and sensitive area, it may be difficult to expect a natural result from an approach that simply adds a large amount of fat.
It is important to proceed carefully after fully considering why the eyelids have become hollow, the extent of the current skin laxity, and which method would look the most natural.
