Hello. I’m Dr. Kim Jun-hyun of Midline Plastic Surgery.
In the previous post, I explained zygoma reduction surgery for people who were concerned about prominent cheekbones.

This time, I’ve prepared an explanation of jaw reduction surgery for those considering mandibular angle reduction because the lower face looks large and wide among facial contouring surgeries.

I’ll use characters to help make the explanation easier to understand.
If the character that comes to mind when thinking of a large, protruding cheekbone is Jigsaw, then the first character that comes to mind for a square jaw that is angular and broad is Thanos.
A developed square jaw gives a strong masculine and robust impression.
However, some women also have a more developed jaw.
Unlike men, women tend to prefer softer contours and a smaller face, so a broad, angular square jaw can cause a lot of stress.
That is why many people come in for consultations, considering jaw reduction surgery to improve their contour line and achieve a smaller face.

Preoperative photos and X-ray
Among those who come for jaw reduction consultations, many receive a diagnosis of square jaw during consultations at multiple contouring clinics because their lower face looks wide and large,
or they come after being recommended square jaw surgery along with chin reduction surgery.
As a result, what started as a consultation for jaw reduction alone often expands to include consultation about the chin as well.
As I always say, the outcome of surgery depends on receiving an accurate diagnosis and surgery from a skilled doctor.
I will explain using a patient at our clinic who came for a jaw reduction consultation and underwent square jaw reduction, jawline treatment, and chin liposuction together.
Please look at the external photos and X-ray images above.
On the X-ray, the lower jaw is large and wide, but from the outside, the square jaw bone may not be very visible because the skin and soft tissue are thick.
Here, soft tissue includes fat.
In such cases, even if square jaw surgery is performed, the effect does not show well because the tissues covering the bone are thick.
Left: a patient whose broad, angular bone is visible / Right: a patient who has broad, angular bone but it is not visible externally.
To help you understand a bit better,
I am comparing a patient whose square-jaw bone is visible with a patient whose bone is covered by soft tissue (fat).
Left: a patient whose broad, angular bone is visible / Right: a patient who has broad, angular bone but it is not visible externally.
So in the case of the patient on the right, if the goal is to improve the wide, large lower face, I explain that liposuction is absolutely necessary along with jaw reduction surgery for there to be a meaningful effect.
Because thick soft tissue means that even if the bone underneath is reduced, the external satisfaction will be low,
it is like changing a thick blanket to a thin one.

Photos and X-rays after combining square jaw surgery and liposuction
For people with a developed square jaw,
jaw reduction surgery is unquestionably a necessary procedure.
No matter how much you lose weight, if the contour itself is large,
it will not become any smaller beyond a certain point.
However, if the face also has a lot of soft tissue,
I hope that after taking X-ray images and external photos during the consultation, you receive an accurate diagnosis of the cause of the wide, large lower face in facial contouring
and undergo effective surgery using the method that suits you.

These are photos of a patient who had square jaw reduction and chin-line liposuction for a double chin performed by me at Midline Plastic Surgery.
Left: before surgery / Right: 7 days after surgery.
On the X-ray, you can see the area where the angular bone has been improved.

Left: before surgery / Right: 7 days after surgery.
And even though there is still postoperative swelling on day 7, the reduced soft tissue around the jawline and double chin makes the jawline look sharper.
If the patient had only undergone square jaw reduction surgery, as diagnosed by another clinic, there would have been changes on the X-ray,
but the external changes would have been limited, which would likely have been disappointing.
Before surgery, I explain to patients the results that can be achieved through facial contouring surgery and the limitations caused by soft tissue (fat).
Some people say they will reduce the fat on their jaw by dieting after bone surgery for the square jaw.
Reducing the bone through square jaw reduction and reducing the soft tissue through liposuction are both options for achieving results.
It was a time to explain that while the difficult and exhausting process of losing weight can be done together with square jaw surgery by also having liposuction,
the effect can be dramatic.
Thank you.

Last week, I took a photo of the hall because it looked beautiful at an acquaintance’s wedding.
Every time I go to a wedding hall, it makes me think of my own wedding too haha

For those considering jaw reduction surgery because they are worried about having a large face,
I conclude this post by wishing for satisfying results through the right surgical method and an accurate preoperative diagnosis.
