Today, I’d like to introduce a case in which a patient visited us because of discomfort in the chin after some time following chin implant surgery.

Chin advancement surgery and double chin, jawline liposuction after chin implant removal
For people with a small lower face,
the chin has little volume, so they worry about a weak chin,
and want to add volume to the chin,
but bone surgery feels intimidating.
So they try filling it with filler,
and then, going one step further beyond the limits of chin filler,
they also consider inserting a chin implant
in order to improve a weak chin.
This patient also worried about having a small lower face,
and said that 7 years ago, after making a big decision, they had a chin implant inserted
to improve their weak chin.
“At first, it was fine.....”
Almost everyone who gets a chin implant because of concerns about a weak chin
starts by saying, “At first, it was fine...”
This patient also felt fine at first after the chin implant,
but later developed discomfort in the chin area and visited after worrying about it.
To check the exact current condition of the chin,
we looked at the CT scan.


The chin implant had shifted diagonally, bone resorption was occurring, and bone was growing over the implant from above and below.

Although you can see the outline of the implant on the CT,
to explain it more intuitively,
I marked the area where the tip of the implant, which had shifted upward to the right,
was stimulating the inferior alveolar nerve
in the photo below.

The tip of the implant, which had shifted upward to the right, is irritating the inferior alveolar nerve.
The area indicated by the yellow arrow is the inferior alveolar nerve,
and the pink dotted line is the chin implant.
The chin implant had rotated diagonally,
and it appears that the right inferior alveolar nerve
is being stimulated.
And since the chin had been under constant pressure from the implant since the surgery,
the resulting bone resorption was not surprising.
But....
As that implant was eroding the bone,
bone was growing over the implant from above and below.

Bone erosion
When foreign materials such as implants (nose, breasts) or fillers enter the body,
the body recognizes them as foreign substances
and forms a capsule (also called a capsule).
It is a response that tries to minimize the effect on nearby tissues inside the body.
In this case, a capsule had initially formed around the implant,
but as the chin bone was resorbed,
bone was growing over the implant from above and below.

After removing the implant,
we spent extra time grinding down the bone
that was surrounding the chin implant from above and below.
This was a case that took longer because of that.
The patient had a double chin,
and because the lower face was small, it made it look even more pronounced.

Even just chin advancement surgery can improve a double chin,
but if double chin liposuction is performed together,
the satisfaction can be much higher, so we proceeded with both.

Even with only the pre- and post-operative X-rays,
you can see the changes in the double chin and the silhouette of the chin.

The chin also improved to a more natural line than when the implant was in place,
and the discomfort in the chin disappeared ^ㅡ^/
