Facial contouring is a surgery that
makes the facial skeleton itself
smaller.
The cheekbone, square jaw, and chin are combined
and are commonly called the three major contouring procedures.

When the cheekbones protrude,
the angles and width of the square jaw are developed,
and the chin is long and projects forward,
it is a procedure with high satisfaction.
However, among contouring surgeries,
except for chin advancement surgery,
all of them reduce bone size,
so the skin inevitably descends slightly after contouring surgery.

Sagging after contouring
The direction is inward and downward relative to the center of the face,
and this is a natural course after contouring surgery.
Please understand that this is a different situation from cheek sagging caused by nonunion of the cheekbone.
This time, a patient came after receiving the three major contouring procedures at another hospital
and wanted to resolve the double chin caused by sagging after contouring.
This is not meant to criticize surgery at another hospital.
I am simply saying that after contouring surgery,
everyone can experience some degree of sagging,
so please do not misunderstand.


Double-chin sagging
On the X-ray, you can see the plates and screws
that fixed the cheekbones and chin.


The plates from the other hospital were not mosaicked.
Even on X-ray, when you look at the side profile,
you can see the silhouette of a double chin.
We decided to resolve the double-chin sagging after contouring
with a face lift
and remove the plates and screws as well.

When performing a face lift,
if the plate on the zygomatic arch is removed,
there is an advantage in that
there is no need to leave a separate scar,
including stitches,
on the sideburn area.

It was explained that a face lift
is not a surgery that creates results by pulling the skin.
The SMAS (Superficial Musculo-Aponeurotic System),
located between the skin and the facial expression muscles,
must be dissected and the retaining ligaments cut.
The key is to pull and fix this flexible SMAS.
Our method can be seen in the pink box above.
Here, the SMAS layer refers to
a very thin fibromuscular layer
between the deep subcutaneous fat layer and the muscle layer in the face,
and it also serves to connect and support the skin and muscles.
As the SMAS layer ages,
skin elasticity decreases,
sagging occurs,
and wrinkles form.
If the SMAS lift is not done properly,
it is difficult to expect a lifting effect.
When the SMAS is dissected,
the deep buccal fat is also exposed,
so it is removed when necessary.
And the fat pad can also be lifted and suspended.

After the face lift surgery, the double chin disappeared,
and the downward sagging from the corners of the mouth,
which looked like jowls,
also improved.
The scar in front of the ear is also very good,
and as time passes, it gradually fades from pink → brown → white.
There was no need to make a separate incision in the sideburn area;
after dissecting the skin,
the plate was removed.

Even on the postoperative X-ray,
you can confirm the silhouette
with the double chin gone.
I explained that sagging that occurs after contouring
can be improved
through a face lift.
