There are many inquiries from people who are concerned because their lower face is long.
The surgery that can be done when the lower face is long is
reduction genioplasty.
But surgery that reduces length leaves
the reduced amount of skin behind,
which can create a double chin. ㅠㅠ
The patient below
shows the side-view change after reducing the chin width by 10 mm and the height by 3 mm.

To help prevent a double chin like this to some extent,
a lower-face lift should be performed
together during surgery.
However, even if the length is reduced,
if the chin is advanced,
a double chin may not form.
This time, I will talk about a case in which the chin was long but recessed,
so the length was reduced while also advancing it
to improve both the chin length and the retruded chin.


From the front, the nose and philtrum are misaligned,
and the lower face below the nose is long.
Because the lower face is long, when closing the lips,
there is a lot of tension in the mentalis muscle,
which lifts the lower lip upward.
The mentalis muscle creates walnut-shell- or pebble-like wrinkles
on the chin.
So the botulinum toxin used for this area
is called cobblestone chin Botox.
I will briefly write a post about cobblestone chin Botox later.


For patients with a long chin, reduction surgery usually lowers the height of the chin by about 3 mm.
In the case of the man above, it was reduced by 6 mm
while also advancing it.
Rhinoplasty was also performed
to achieve an ideal profile.

The feeling that the facial axis was misaligned from the front has improved,
and as the height of the lower jaw became shorter,
the long lower face also appears improved.

From the side, the drooping arrow-shaped nose tip was lifted.
With chin advancement, a double chin did not form,
while the long lower face became shorter in height,
and the profile from the nose to the mouth and chin improved.
Cases where reduction is combined with advancement like this
are not common.
In most cases where chin reduction is needed,
the chin tip also needs to be moved backward,
which is the case in many patients.
I am explaining that the diagnosis can differ depending on each individual case.
This was a case that showed how much the side profile can be improved
by reducing a long lower face,
not forming a double chin through advancement,
and performing rhinoplasty at the same time.
