Midline Plastic Surgery
The patient below had a short, small lower face
and visited the clinic because of concerns about a receding chin.
When the lower face is small, advancing the chin forward can improve both the length of the chin in the front view,
which had looked short, and the blunt-looking jawline in the side view at the same time.
So chin advancement surgery has dramatic results,
and it is one of the surgeries that patients are very satisfied with at follow-up visits.

When the lower face is small and retruded,
it is not easy to notice whether the chin is asymmetric.
For example, it is somewhat like how a dot on the face is more noticeable when it is large than when it is small.

If a receding chin that is small, short in length, and positioned backward is treated by planning only advancement genioplasty,
the asymmetry of the lower face that was not noticeable before, and the curvature of the chin,
will appear and will not be resolved.
If you look closely at the yellow arrow, you can see that the chin is pointing to the left.
The reason I named the hospital
“Midline Plastic Surgery”
was because I also placed importance on aligning the midline.^^

I put a lot of thought into the logo as well,
so that the center line would be emphasized.^^
Returning to today’s topic,
I will explain how we improved a chin that was small, short, retruded, and asymmetrically curved to the left.
As I mentioned above, the preoperative diagnosis and the resulting surgical plan are the most important parts.
In this patient’s surgery,
when the chin bone was cut and fixed, moving it 1 mm to the right during fixation was the key point in improving the asymmetry.

A saw-shaped osteotomy cannot move the chin left or right.
If you have a curved chin and are worried about asymmetry,
a “saw-shaped osteotomy” cannot create symmetry.
That is because a “saw-shaped osteotomy” cannot move the chin left or right.

With a T-shaped osteotomy, the width can be reduced while also allowing left-right movement.
At our clinic, we almost never perform a saw-shaped osteotomy.
It is used only when the inferior alveolar nerve is positioned very low,
and the risk of injury is high,
and even then only with a slight angle.
The basic approach for chin surgery at our clinic is
straight-line (I-shaped) and T-shaped osteotomy.
The advantage of a T-shaped osteotomy is that it can reduce the width
while also allowing left-right movement.
Using the method above,
these photos were taken 14 days after advancement genioplasty.

By moving the left-turned chin 1 mm to the right,
you can see that it has been aligned to the center.
Even though it is only 1 mm,
that is the symmetry achieved by placing the chin on the midline, that is, the center line.
The 1 mm difference matters a lot in this case.
You can see it just by looking at the yellow arrow.

In the side view of the face, asymmetry was not visible,
but the short, small, retruded chin
was advanced forward and changed into a more three-dimensional appearance.
The profile—from the tip of the nose to the lips and the chin—changed in a satisfying way.

As I always feel,
chin advancement surgery makes me think every time that
there may be no better surgery for improving the side profile than this.^^
Today, following last time’s
"non-surgical symmetry correction,"
this was a session on
"surgically correcting symmetry."
If I have the chance next time,
I will prepare a case that corrects symmetry with fat grafting as well.
