About Micro Fat Grafting for the Upper Eyelids

Q. If there is no fat above the eyes,

what changes can occur?

A. A double fold can form in the upper eyelids,

making the double eyelid line unstable,

causing discomfort when opening the eyes,

and making the eyes look sleepy, as if there were ptosis.

On the other hand, it can also create a Westernized look,

so there are people who prefer sunken eyes.

However, for East Asians, it seems to make the eyes look tired.

In the morning and before noon,

the upper eyelid hollowness may not be noticeable because of swelling,

but in the evening, when the swelling goes down,

the upper eyes become hollow and multiple lines

like a double eyelid may appear. Surprisingly, there are many people like this.

Sallow Eyes

In the previous post, I showed how to correct cases where the upper eyelids are sunken and create double double eyelids.

For people who have no volume above the eyes,

the eyelid skin is that much thinner,

so even when performing micro fat grafting,

it must be done more delicately and carefully than regular micro fat grafting,

by making the fat particles smaller and grafting them "in very small amounts"

so that the fat does not look lumpy above the eyes.

Micro fat grafting is a surgical technique that reduces tissue damage and increases the take rate by evenly injecting small fat particles layer by layer into multiple layers, allowing blood vessels to form easily between the small fat particles, while also lowering inflammation and necrosis rates.

And the amount of transplanted fat that takes,

as well as the rate at which it takes,

differs from person to person,

and once it has taken,

it becomes part of the face.

This is a major advantage

that fillers cannot offer.

However, this can also be a disadvantage.

When too much fat takes because of overcorrection,

it is not easy to remove.

Advantages of Micro Fat Grafting

For that reason, I think fat grafting

should be performed conservatively.

And when grafting fat into sunken eyes,

because the area where the fat is grafted has thin skin,

I recommend receiving treatment from a board-certified plastic surgeon

with extensive experience in sallow eyes and upper-eyelid fat grafting.

The patient in the photos below

had no volume in the upper eyelids,

and the eyelid skin was even thinner than that of the patient in the previous post.

Also, the under-eye fat was protruding,

and there was no volume above or below the cheekbones,

Because there were many hollow areas,

full facial fat grafting was needed,

so the required fat was harvested from both thighs.

If fat were removed from only one thigh,

the thickness of the two thighs could become asymmetrical.

And one more thing! Fat grafting is important, but harvesting is important too.

At our clinic, we use a specially custom-made

long cannula when harvesting fat.

Q. Why is the length of the harvesting cannula important?

A. If the length is not sufficient,

fat may be harvested only from the groin to the middle of the thigh,

leaving the area below the middle of the thigh

looking relatively fuller,

which can result in an unnatural-looking thigh shape.

And when the amount is small

and fat is harvested from only one side,

we also use a long cannula

so that the harvesting is even and, even if the amount is small,

Fat Grafting Recovery Period

Now that I have explained the procedure,

I will also explain the recovery process after surgery.

The surgery takes about 30 minutes from harvesting to grafting,

and is performed under sedation anesthesia / with same-day discharge.

There are no stitches on the face,

but there are stitches at the fat-harvesting site (if from the abdomen, one or two stitches at the navel;

if from the thighs, one or two stitches in the groin),

which are removed on day 7.

Bruising and swelling vary from person to person,

but since the upper eyelids are an area that bruises easily,

we recommend stopping supplements related to blood circulation 7 days before surgery.

Micro fat grafting can be performed not only for sunken upper eyelids,

but also for the tear trough.
