How can deep wrinkles on the forehead be improved?
It is generally known that deep forehead wrinkles appear as we age.
That is true.
However, in my school days, I had friends with deep wrinkles on their foreheads.
I’m sure some of you remember one or two such friends,
and I thought some of you might even be the ones with those wrinkles, so I prepared this post.

All wrinkles, not just forehead wrinkles,
form perpendicular to the direction in which the muscles move.
Because the forehead muscle raises the eyebrows upward,
the wrinkles form horizontally.

If those deep horizontal forehead wrinkles continue for years,
they become indented like scar tissue, and even without applying force,
they remain sunken in.
At that stage, it can be said that Botox alone cannot remove the wrinkles.
That is why I always say wrinkle care should begin before wrinkles settle in.
If there are already established wrinkles, then the role of Botox afterward
is only to prevent those settled wrinkles from getting deeper.
Even so, it is certainly better than not managing them at all.
The reasons forehead wrinkles can appear even at a young age
can be divided into two cases.
The first is when the strength to open the eyes is insufficient.
The second is when the forehead is thick and heavy, pressing down on the eyes from above.
In both cases, the person feels visual discomfort,
and as a result, uses the forehead muscle, an auxiliary muscle that helps lift the eyelids.
First, when the strength to open the eyes is insufficient, the solution is to compensate for that strength through eyelid correction.
There is a post explaining when incisional eyelid correction is needed,
so I hope you will find it helpful. ^^

Second, when the forehead is pressing down, a forehead lift alone can help.
Now, even in the first case, where eyelid correction is needed,
there are people who have used their forehead heavily for a long time, resulting in deep forehead wrinkles.
And if the distance between the eyes and the eyebrows is also narrow, then before eyelid correction,
a forehead lift may be needed to widen the distance between the eyes and eyebrows while smoothing the forehead.
In the second case, simply performing a forehead lift
improves the visual field and makes opening the eyes easier.
It is good when the forehead is smoothed out like this, but wrinkles that have been indented for years or even decades
may not fully flatten out. ㅠㅠ
At this point, filler can help make the wrinkles less noticeable.
A male patient in his early 20s visited the clinic.
He was concerned about forehead wrinkles that he had had since childhood.

In the photos, the wrinkles are marked with fluorescent light-green arrows,

The patient had thick skin and had lived with the forehead pressing on the eyes,
so it had become a habit to lift the eyebrows in order to open the eyes.
When I lifted the eyebrows by hand, he said it felt easier to open his eyes, and he also liked how his eye shape looked,
so we planned a forehead lift.
The two clearly sunken wrinkle lines (deep forehead wrinkles) really catch the eye.
These wrinkles are still visible even after a forehead lift.
So I explained filler as a way to push the wrinkles up and smooth them out,
and we decided to do it together with the forehead lift.

Before and after surgery, the forehead became smoother and the eyes looked more refreshed,
and along with the added volume of the forehead, the wrinkles became barely noticeable.
At the consultation, the patient came with the forehead covered,
but for the follow-up visit, he came with a hairstyle that showed the forehead. ^-^
I shared a case where a better result was achieved by combining a forehead lift with filler
for forehead wrinkles.
