For those who are worried about brow asymmetry, this post
may offer a solution.
Alright, let’s get started.
This is a patient I previously introduced for entropion.
If you have a good eye for detail, you may have noticed that the brow asymmetry and uneven eyes improved before and after surgery. ^-^

The patient wanted the double eyelids to show more. However, since the space between the eyes and the brows was narrow, if we raised the crease line with double eyelid surgery,
the distance between the eyes and brows would become even narrower, and the eye expression would look stronger.
In addition, the space between the right eye and brow was already narrower, and the crease was smaller, so
if we tried to make that crease more visible by removing more skin, compared with the opposite side (left),
the distance between the eyes and brows would become even narrower.
The reason for this asymmetry is that the right side of the face, meaning the bone structure, is smaller than the left.
Conversely, you could say the left side is larger than the right.
When the bone is smaller, the brow sits lower than on the larger side,
the eyeball is positioned slightly deeper, and the eyelid covers a bit more.
This causes the eye to be less exposed, meaning the eye appears smaller than the opposite side.
Since we cannot change the size of the bones, we can try to reduce the asymmetry by adjusting brow height.
In other words, the lower brow is overcorrected, and the higher brow is undercorrected.
I have mentioned before that fixation during a forehead lift requires overcorrection, and that this is how it should be done.
However, for asymmetry correction like this, there will be a side that is undercorrected.
This patient wanted
- more visible double eyelids,
- improved asymmetry, and
- a narrower-looking forehead.
All of this was possible in this case, so we planned an endoscopic forehead lift and forehead reduction.
Saying that everything was possible means that there are cases where it is not.
For example, there are cases where the right side of the face is smaller, so the right eye is also smaller, but the brow heights are similar.
In that case, pulling the right side more would make the eye sizes similar, but it would create a brow asymmetry that did not exist before,
so both sides need to be pulled similarly.

We planned an endoscopic forehead lift and forehead reduction as the method to improve brow asymmetry, asymmetry in the double eyelid crease, and a wide forehead.
The brow on the right was lower, and at the same time the right double eyelid crease was smaller than the left, so
we performed the forehead lift with a difference in pulling force, pulling the right side more.
Since the forehead was already wide before surgery, if we performed a forehead lift, the hairline of the broad forehead would
move upward even more, so we also performed forehead reduction (the hairline is brought downward in the direction of the yellow arrow).
This is the result 3 months after surgery.

I tried to match the photos to the same scale, but it wasn’t easy^^;;
As the brow heights were balanced, the double eyelid crease also became similar.
The forehead that looked wide now has better proportions, giving a feeling that it is neither wide nor narrow.

The changes in brow height, double eyelid crease, and forehead width are clearly visible.
The forehead reduction scar at 3 months is also satisfying.

The scar reaches the maturation stage after 6 months, and the pigmentation fades and it becomes softer.
For those considering forehead reduction because they have a wide forehead,
you do not need to worry too much about the scar. ^-^
