Hello, this is Dr. Kim Jun-hyun from Midline Plastic Surgery.
Under-eye fat makes a person look tired and older.
So for people who do not have time for surgery or are afraid of surgery,
filler is sometimes injected under the eyes as a supplement.
However, simply filling only the tear trough, which is the hollow area beneath the protruding under-eye fat,
without addressing the bulging under-eye fat is nothing more than a temporary fix.
Filler will be absorbed to some extent,
and then the protruding fat will become visible again.
Moreover, if filler is added under the area where the under-eye fat is already excessive,
it can also make the under-eye area look swollen.
This patient was concerned about under-eye fat.
They said they had previously filled the tear trough with filler because they did not have time for surgery.

The tear trough is not very pronounced, but the under-eye area looks puffy, which is disappointing.
In addition, under-eye fat is visible on the left side.
In this condition, surgery is not performed right away.
That is because we do not know the condition of the under-eye fat or how deep the tear trough is.
We first dissolve the under-eye filler. It is best to do this at least 7 days before surgery.
This is because when filler meets the injection that dissolves it (hyaluronidase), it turns into water, and time is needed for that to be absorbed.
This is how the under-eye filler looks after being dissolved.

Left) Before dissolving under-eye filler / Right) After dissolving under-eye filler
The tear trough that had been filled by the filler is now visible.
The silhouette of the under-eye fat is also clearer.
Now it is ready for surgery.
This time, because the space between the eyes and eyebrows was narrow and the eyes looked crowded,
the patient decided to have a forehead lift at the same time.
This is the result 3 months after surgery.

Left) Before under-eye fat repositioning, fat grafting, and forehead lift / Right) 3 months after surgery
Under-eye fat repositioning was performed together with under-eye fat grafting.
The tired look disappeared, and the puffy appearance was also improved.
Looking at the result of the forehead lift, it was definitely a necessary procedure, right? ^-^
The less under-eye fat and tear trough there are, the better.
That is because it makes the area around the eyes look brighter, which greatly helps improve the overall impression.
For those who were afraid of surgery or had no time and chose filler instead,
the puffy under-eye fat that filler could not hide
can be addressed with filler dissolution and lower eyelid fat repositioning surgery.
