Hello, I’m Dr. Kim Jun-hyun of Midline Plastic Surgery.
I previously explained the secondary angle after chin advancement surgery.

If you have not read the post above,
or if you have seen reviews from other clinics mentioning a bone step after surgery and feeling stressed about it,
I can imagine that chin advancement may seem scary and worrying, and I completely understand that.
Let me ease those concerns by showing you another case.
I take X-rays and CT scans one week after chin advancement.
Naturally, because the bones have not yet fused, the space between the bones appears separated,
and this is the period when the secondary angle is especially noticeable.

The area marked with the yellow circle is where the osteotomy begins,
and a step appears by as much as the amount of advancement.
This patient came in three years later for plate removal.

This is a panoramic image. The left side is one week after surgery, and the right side is three years after surgery.
You can see that the step seen in the yellow circle has become smoother in the green circle.
Just as a sharp rock becomes smooth when waves hit it,
as bone resorption and fusion take place, the step disappears or improves to the point where it is no longer noticeable.
Now let’s look at the 3D CT image.

The yellow circle on the left is one week after surgery, and the green circle on the right is three years after surgery.
The 3D CT is even more convincing than the panoramic image, right?^^
We looked into the fate of the secondary angle created by chin advancement.
