As aesthetic standards have shifted closer to Western features, there has been a growing preference for larger, more defined eyes and clearer facial features. In Korea, because of the characteristics of East Asians, many people have single eyelids, and eyes that are short vertically and may look somewhat cramped. As a result, interest in eye surgery has naturally increased in order to create a more attractive image. When many people look into ways to make small eyes look more distinct and defined, it is important to carefully learn about the procedures that are needed, because there are various methods for making the eyes appear larger, such as eye correction surgery and canthoplasty, in addition to double eyelid surgery. Today, we are going to take a look at whether double eyelid surgery and canthoplasty really need to be performed at the same time.

Do people usually get double eyelid surgery and canthoplasty together?
Double eyelid surgery and canthoplasty are among the procedures that are very commonly done together, and at our clinic, about five out of ten patients undergo them at the same time.


Even double eyelid surgery alone makes the eyes look bigger.
So why do people also get canthoplasty?

Double eyelid surgery and canthoplasty work in different directions, so they are often performed together.

You can think of double eyelid surgery as making the eyes larger upward, while canthoplasty makes them larger horizontally.


You may think that making the eyes bigger both upward and sideways will give you the large, clear eyes you want, but canthoplasty can cause various side effects, so balance and harmony with other features are important.


What is the most common canthoplasty procedure done with double eyelid surgery?

It would be inner canthoplasty.
This is because most Asians have the Mongolian fold (the skin covering the inner corner of the eye), so inner canthoplasty is often performed to remove the epicanthal fold.
Inner canthoplasty is a procedure that removes the epicanthal fold at the inner corner of the eye to open up the front of the eyes more clearly, while upper canthoplasty is a procedure that removes the skin covering the front part of the double eyelid at a 45-degree angle.

When double eyelid surgery is performed, there is also an effect of making the outer part a little wider. Because this can also address sagging on the outer side, outer canthoplasty is often not essential.
Outer canthoplasty extends the short horizontal width of the eyes outward, while lower canthoplasty adjusts the angle of the outer corner of the eye.



Is there a reason to perform double eyelid surgery and canthoplasty at the same time?

At our clinic, we do not necessarily recommend canthoplasty and double eyelid surgery together. However, when it is aesthetically not ideal, we do recommend inner canthoplasty.


If the epicanthal fold is severe, we recommend inner canthoplasty. For example, if a patient wants a high double eyelid line but the epicanthal fold remains, the inner corner of the eye may look closed off and unnatural. In such cases, performing inner canthoplasty at the same time can release the epicanthal fold and secure the height of the line, leading to a more natural result.

On the other hand, for people who truly do not want canthoplasty, the procedure should proceed as a practical compromise. The double eyelid line should not be set too high, and the design should avoid an awkward bend in the front part of the line.
And if, over time, you decide that canthoplasty is needed, you can have it performed later.

Does the canthoplasty design change depending on the double eyelid line?

Because East Asians, in particular, often have a severe epicanthal fold, the double eyelid line can be chosen as an inline, out-line, or in-out line depending on that. Therefore, canthoplasty can be a very important factor. A suitable design is applied depending on how much the epicanthal fold is opened, which makes a natural result possible.
If there is no epicanthal fold, the shape of the double eyelid line can be decided by considering the overall proportions and harmony.




When you end up with an unattractive canthoplasty result

It can happen when it is opened too much, when the scar opens and becomes a sunken scar, when aftercare is done incorrectly, or when the fixation during surgery is weak.
Because of these various side effects, it is necessary to carefully check everything before undergoing canthoplasty.


How can side effects be prevented?

As double eyelid surgery and canthoplasty have increasingly been treated like a set, many people have begun to take them lightly. However, it is important to remember that revisions are increasing because they were chosen too casually.
When looking at the majority of people who come to our clinic for revision surgery, many are considering revision because of problems and side effects after pursuing overly aggressive results. If double eyelid surgery and canthoplasty are performed at the same time,
can everyone get a dramatic result?

Many people often think that double eyelid surgery, eye correction, and canthoplasty all make the eyes bigger, but not every case becomes bigger.
It can vary depending on the eye condition.
If the eyes are deep-set, the eye socket is small, and the bones around the eyes are close, canthoplasty would be unnecessary. If the eyes are slightly protruding and the epicanthal fold is severe, then even a small amount of opening can give good results.


In the case of small eyes, there are many cases where the eyes become much more dramatic when canthoplasty or eye correction is performed at the same time. However, the procedure must stay within a range that is not too excessive in order to achieve the beautiful eyes you want.
