Hello.
This is Iho Plastic Surgery, the place where you can become beautiful again.
There are a variety of aesthetic standards, but if you feel that your eyes look sleepy or tired and this affects your relationships with others or causes social stress, you may consider double eyelid surgery to try to address it. However, it is not always a sufficient solution. If the problem is simply sagging eyelids, improvement may be possible with double eyelid surgery or upper blepharoplasty. But if the muscles that open the eyes are weak, that alone is not enough, so a specialist must examine and diagnose the condition and apply the correct method to achieve the desired change.
So, let’s take a look at whether there is a way to diagnose what is meant by weak eye-opening muscles and what solutions are available accordingly.

What is ptosis?
Ptosis can be classified according to how much of the black of the eye is covered.

If the eyelid covers 1 to 2 mm of the black of the eye, that is considered within the normal range. Based on that, there are three criteria for determining whether or not it is ptosis.

Criteria for classifying ptosis
Normal range
The upper eyelid covers about 1 to 2 mm of the black of the eye.
Mild ptosis
The black of the eye is covered 1 to 2 mm more than the normal range.
Moderate ptosis
The black of the eye is covered about 3 to 4 mm more than the normal range.
Severe ptosis
It droops more than 4 mm below the normal range and covers the pupil as well.

What are the symptoms of ptosis?

Because ptosis causes the pupil to be covered, you end up using your forehead a lot to open your eyes. As a result, the forehead muscles are used more, which can cause wrinkles. So even at a young age, deep wrinkles may make you look older.

In cases of ptosis, vision may decline because the pupil is covered. If the eyebrow is poking the eyeball, it is better to receive correction as soon as possible. You may hesitate to undergo surgery because you are too young, but for vision protection, we may recommend doing it early.

It is good to remember that the eyes are an area that serves both a functional role in seeing objects and an aesthetic role that affects first impressions.
If you have ptosis and struggle to open your eyes, so you use your forehead a lot, or if you often complain of headaches, or if people keep telling you that you look sleepy even when you are not tired, then resolving that complex and regaining confidence can be one option.

What surgical method is used to improve ptosis?

You should not assume you have ptosis just because you use your forehead a lot. That is why we recommend visiting a plastic surgery specialist at our clinic in person, receiving a thorough consultation, and then making a decision.

The surgical method used to address ptosis is eye correction surgery.

Broadly speaking, eye correction surgery can be done in two ways: a fixation method and a cutting-and-reconnecting method. Which method is applied is determined by evaluating the strength of the eye-opening muscles, the thickness of the skin, and the amount of fat.

Can ptosis occur on just one side?

Ptosis can occur on only one side for various reasons, such as a problem with the muscle origin, a nerve-related issue, congenital factors, or a tumor in the brain or eye. Therefore, ptosis can appear in only one eye.

If ptosis occurs in only one eye, the degree of fixation of the eye-opening muscles is adjusted differently to balance both sides.

Can it come loose after surgery?

Yes, it can.
In eye correction surgery to improve ptosis, a common method is to attach the tarsal plate and the eye-opening muscle together.

Because this is done by tying things together, the thread can come loose. Even if it was fixed to the tarsal plate, the thread may loosen from the tarsal plate, or it may be loosened without you realizing it after some kind of impact while sleeping. As such, it is good to remember that loosening is possible for a number of reasons.

It usually does not come loose easily, but to help prevent this in advance, the tarsal plate can be pierced at an appropriate thickness and the eye-opening muscle can be secured with proper tension to prevent loosening. Therefore, it is best to meet with medical staff who thoroughly understand various factors such as the thickness of the individual’s eyelids and the degree of eyelid sagging, and who can reflect the appropriate method accordingly and predict the changed appearance after surgery.

Ptosis
Ophthalmology? Plastic surgery?

Ptosis surgery is often performed in plastic surgery.

Most surgical cases are handled in plastic surgery. If you are choosing between a medical team that has handled many different cases and one that has not, it can be said that choosing a team with more experience increases the likelihood of a more satisfying outcome. The eyes are a very important part of the body functionally, as they move all day long to protect the eyeball. Of course, to achieve a more favorable impression, personalized surgery that considers the surgical method and design must be performed, so it is important to meet medical staff with a high level of anatomical understanding and a strong sense of aesthetics.
