Wrinkles form during this process, and the area around the eyes is no exception. The eyes are one of the most influential areas of the face when it comes to changes in appearance, so even small changes can make a big difference in overall impression and can make you look like a different person. For this reason, many people have begun to take an interest in plastic surgery in the hope of looking a little younger. Unlike before, when the skin above the eyelids sags and droops, it can make you look older, which leads many people to become interested in a more youthful appearance. Because every procedure has its pros and cons, it is best to carefully check the condition of the eyes to determine whether upper blepharoplasty or a brow lift is appropriate, and then proceed with the right method.

For example, if removing only the eyelid skin is enough to create a nice double eyelid while improving the sagging area, and if there is asymmetry in both eyes or if the eyes need to be opened a little more distinctly, then upper blepharoplasty can address these issues.

If the skin is a little thick, the eyebrows are positioned low, and the brow bone is somewhat prominent, then double eyelid surgery or upper blepharoplasty alone may not improve the drooping eyelids very much. In such cases, a brow lift is a better option.

Please explain the differences between the surgical methods.

First, upper blepharoplasty can be understood as a procedure that removes a significant amount of sagging skin while performing double eyelid surgery.

Double eyelid surgery falls within the category of upper blepharoplasty, and it is a method that addresses sagging by removing the drooping skin and a certain amount of muscle along the double eyelid line.

A brow lift involves making an incision along the boundary between the eyebrow and the upper eyelid and removing thick skin on the outer side of the orbit to address the sagging area.

Let’s explain with case examples.

First, a patient in their mid-30s addressed drooping eyes through upper blepharoplasty.

In the pre-surgery photos, you can see that they had multiple eyelid folds, and through upper blepharoplasty, the sagging eyelid skin was removed to create a single line, resulting in a clearer eye shape.

You may be wondering why, despite the patient being in their mid-30s, the diagnosis was not double eyelid surgery but upper blepharoplasty.

It was because of the loosened skin.

At first, the eyes had multiple eyelid folds, and it was possible to see that the upper eyelid area was somewhat heavy.

In cases like this, regardless of age, it is possible to identify during an examination that there is either naturally abundant eyelid skin or significant skin laxity. In order to change the eyes into a clearer and more defined shape, excision is unavoidable, so it falls under the category of upper blepharoplasty, and that is why the diagnosis was upper blepharoplasty.

The second case is a woman in her mid-50s who underwent upper blepharoplasty.

In her case, she wanted to improve the sagging area while also creating a double eyelid line, so we accommodated her needs and proceeded with upper blepharoplasty.

The double eyelid height was set at 7 mm, and even without performing ptosis correction at the same time, you can see that the eyes look more refreshed.

As a third case, a woman in her early 40s underwent a brow lift.

She already had a double eyelid line, but as the skin sagged with aging, the double eyelid line became hidden under the skin and could no longer be seen.

Because the amount of skin in the area was relatively large and the sagging was severe, if we had performed excision through upper blepharoplasty, too much tissue would have had to be removed. This could have caused the thick skin to descend and made the eyes look unnatural, so we proceeded with a brow lift. The patient also liked and was satisfied with her existing double eyelid line, so she was pleased with the result of preserving that line through the brow lift.

Lastly, a woman in her 50s underwent a brow lift and buried suture double eyelid surgery.

In her case, the procedure was not upper blepharoplasty but double eyelid surgery. The reason was that a brow lift alone could sufficiently remove the sagging area, so no excision was needed during the double eyelid procedure, and the double eyelid line was created with buried sutures.

As in the last case,

even if drooping eyelids are improved through a brow lift,

you can still have double eyelid surgery at the same time.

Although upper blepharoplasty and brow lift are both procedures that remove drooping eyelid skin, their purposes differ slightly, so depending on the patient’s eye condition, the two surgeries can be performed at the same time.

However, since these are procedures performed on the upper eyelids at the same time, you should also carefully consider side effects such as asymmetry and make your decision after thorough thought.

To expect a satisfying result,

the surgeon’s experience is more important than anything else.

Because this involves sagging that appears after middle age, there are many accompanying issues.
