Today, I would like to talk about a case involving a woman in her early 30s who had received nasal filler several times in the past and then underwent nose surgery after filler removal.

When you want to raise a low nose but feel hesitant about nose surgery, nasal filler is something many people choose.

Because filler procedures are simple and the recovery period is short, it is one of the procedures people often select with less hesitation.

However, fillers usually begin to break down gradually in the body after about 6 months to 1 year, and the shape starts to loosen.

Many people believe that filler is absorbed and disappears, but rather than disappearing, it loses its shape and only the volume remains.

So a few months later, people get filler again, and as time passes and the shape loosens, they repeat the filler procedure. This can cause the bridge of the nose to look puffy and awkward.

This patient also had nasal filler about four times, and as a result, she had received several different types of filler. Because of this, she came to the clinic with a nose that looked spread out and had become lumpy on one side.

Preoperative diagnosis

From the front, the bridge of the nose looks spread out and especially lumpy on one side.

It is likely that during the repeated filler procedures, an attempt was made to correct the original crooked shape of the nose by injecting more filler on one side, and that this process was repeated multiple times, which likely led to these symptoms.

In addition, the nasal tip is not well defined. Even though nasal filler was performed several times, it would have been difficult to achieve the desired nasal tip shape. The alae are also somewhat wide. From the side and at a 45° angle, the glabella area has been raised, but the height and shape of the nasal tip are still somewhat disappointing. The overall line of the nose is not very feminine.

Surgical process

Usually, during nose surgery, an injection to dissolve the filler is administered one week before surgery, but at our clinic, to shorten the recovery period, we perform the filler-dissolving process at the same time as surgery.

Because this patient had received several types of filler, some of the dissolvable filler was partially dissolved by injection, but in the case of non-dissolvable filler, we removed it directly during surgery.

The bridge of the nose was raised using an appropriately sized silicone implant, and the nasal tip was shaped using septal cartilage and ear cartilage.

Alar reduction was also performed to reduce the wide nostrils.

Postoperative progress

After surgery, the front view shows a clear appearance without the feeling of a spread-out nasal bridge. The crooked appearance caused by the improper filler procedures was also improved. The bulbous look at the nasal tip was greatly improved, and the wide nostrils were reduced, giving a sharper appearance. From the side and at 45°, the line extending from the bridge to the nasal tip has become much more feminine and refined.

Nasal filler procedures are simple and have a short recovery period, so many people undergo them. However, fillers clearly have limitations, and because they have a tendency to loosen over time, one should be careful when choosing this procedure.

It may be possible to have filler done once or twice, but if it is repeated many times, a spread-out nose can occur as the filler disperses. In such cases, rather than continuing filler procedures, it is a better choice to correct it through nose surgery.
