Hello^^ I am Dr. Baek In-soo, a board-certified plastic surgeon and the representative director of Garnet Plastic Surgery.
Today, I’d like to introduce the case of a woman in her 60s who visited us because of under-eye sagging.

Reason for Surgery
She said she had visited several clinics for consultations in order to address the dark, sagging under-eye area she had been concerned about for some time.
She had also seen people of the same age group lose their aegyo-sal after lower blepharoplasty and end up looking very obviously operated on, so she wanted a natural result that would not look surgical!
Preoperative Diagnosis

Let’s take a look at her preoperative condition. The skin under the eyes and the fat pads had lost elasticity, causing slight bulging, and the tear trough under the eyes appeared deeply hollowed.
In addition, the elasticity of the midface below the eyes and beside the nose also appeared reduced. The aegyo-sal created by the orbicularis oculi muscle was also absent.
Surgical Process

First, the 'Quad Plus Lower Blepharoplasty' is an advanced technique developed from the standard lower blepharoplasty procedure. After making an incision in the skin under the eyes and dissecting the tear trough, the fat under the eyes is repositioned and any unnecessary fat is removed.
After that, the orbicularis oculi muscle and fat are lifted through SOOF lifting, and the remaining loose skin is excised before being carefully sutured. Compared with a standard lower blepharoplasty, this approach can create a brighter under-eye area because it includes SOOF lifting and fixation of the orbicularis oculi muscle.
Postoperative Progress

The bulging fat pads and tear trough under the eyes were improved, resulting in a smooth under-eye area.
Through SOOF lifting, the elasticity of the midface was restored, and with suspension fixation of the orbicularis oculi muscle, aegyo-sal was created, helping restore a firm and lively eye area.
You can clearly see that the under-eye area is smoother and brighter than before the surgery, right?

Today, we took a look at lower blepharoplasty for a woman in her 60s.
With the upgraded Quad Plus Lower Blepharoplasty method compared to conventional lower blepharoplasty,
