When patients come in for a consultation about double eyelid surgery, one of the things they ask most often is the design of the double eyelid, such as inline, in-out line, semi-out line, or out line.
Of course, the design of the double eyelid is also very important, but from a plastic surgeon’s point of view, the most important thing is first to check whether the strength used to open the eyes is sufficient, and especially whether there is a difference in that strength between the left and right eyes.

Left-right asymmetrical double eyelids
Usually, when only one side develops a double eyelid,
the side that opens the eyes well has a double eyelid,
whereas the side that does not open the eyes well often has no double eyelid or develops a folded crease.

Difference in pupil exposure between the left and right eyes
The left eye (right side in the photo) opens well, with the pupil exposed by about 80%, but the right eye (left side in the photo) has slightly weaker eye-opening strength, with the pupil exposed by about 70%.
Non-incisional ptosis correction is a method that uses surgical thread to create a double eyelid line while pulling the muscle that helps open the eye, called the Muller muscle.
Because the thread is passed internally, the procedure is performed through a small needle puncture, and ptosis correction is carried out from the conjunctival side on the back of the eyelid.

If the strength used to open the left and right eyes differs, the amount of muscle tightening is adjusted differently to create symmetry.
The right eye, which had weaker eye-opening strength, is corrected more while the double eyelid is formed.

Non-incisional double eyelid surgery and ptosis correction have the advantage of quick recovery, so one week after surgery there is not much swelling or bruising.
Even though it was performed together with facial fat grafting, there is not much swelling around the eyes.

At one month after surgery, there is almost no swelling, and the line looks slightly thinner than it did at one week.
The double eyelid line is also symmetrical, and the ptosis correction on the right eye, which had weaker eye-opening strength, was done well.
Non-incisional double eyelid surgery and ptosis correction have the advantages of a short operation time and quick recovery.
However, non-incisional ptosis correction is not possible in every case.
It is effective in mild ptosis where the skin is thin, eye-opening strength is not too poor, and the difference in pupil exposure between the left and right eyes is within about 5–10%.
