There are several ways to address sagging skin on the upper eyelids.
These include upper blepharoplasty, which is similar to double-eyelid surgery; sub-brow lift, which pulls the skin from around the eyebrows; supra-brow lift;
and forehead lift, which lifts from inside the scalp.

Among these, the most commonly performed procedure is upper blepharoplasty, and it can be considered the 대표 procedure for middle-aged eye surgery.

Surgical Method of Upper Blepharoplasty
Upper blepharoplasty removes sagging skin from the upper eyelids, and in East Asian patients, the surgery is completed by creating a double eyelid, which is why it is sometimes called middle-aged double-eyelid surgery.

Upper blepharoplasty is a good procedure, but because it cuts away eyelid skin, it is important to check the distance between the eyes and the eyebrows before surgery.

Cause of Eyelid Sagging
Sagging skin of the upper eyelids may involve sagging of the eyelid itself, but in many cases it occurs because the forehead and eyebrows droop downward.
Based on my personal experience, about 70% of eyelid sagging is usually caused by forehead sagging, while about 30% is due to sagging of the eyelid itself.

When the Distance Between the Eyes and Eyebrows Is Short
When the distance between the eyes and eyebrows is short, it can create a strong impression. If too much eyelid skin is removed through upper blepharoplasty in this case, the sagging skin may be corrected, but the strong impression can become even stronger.
In such cases, it is better to consider a forehead lift along with upper blepharoplasty.

Skin Removal When Performing Upper Eyelid Surgery with a Forehead Lift
When a forehead lift is performed together, less sagging eyelid skin needs to be removed, making a more natural result possible. Above all, it has the advantage of increasing the distance between the eyes and eyebrows, helping create a softer appearance.
A forehead lift can be performed through a small incision inside the scalp, and because the surgery is done using an endoscope, it has the advantage of a faster recovery.

Forehead Lift and Major Structures

Temporal Vein Seen in the Endoscopic Field

Supraorbital Nerve Seen in the Endoscopic Field

Retention Ligament Dissected in the Endoscopic Field
Endoscopic dissection in forehead lift can significantly shorten the recovery period compared with conventional non-endoscopic lift surgery, so even when performed together with upper blepharoplasty, there is not a large difference in recovery time.
And most importantly, it can soften an impression that might otherwise appear harsher after upper blepharoplasty.
Let us compare the before-and-after results at 2 weeks after surgery.

Change in Eyebrow Position Before and After Surgery
Because a large part of the eyelid skin sagging was addressed through the forehead lift, upper blepharoplasty became possible while widening the distance between the eyes and eyebrows and making the eyelids feel lighter.
And even though the photos were taken 2 weeks after surgery, you can see that there is no significant swelling or bruising.

Comparison Before and After Surgery
There are several surgical methods for addressing sagging of the upper eyelids, and it is important to choose the right procedure according to the situation.
