The area that receives the most plastic surgery is eye surgery, including double eyelid surgery, and the second most common is nose surgery.

Although the younger population is decreasing, the surgeries most often considered by people starting their first plastic surgery in their 20s are eye surgery and nose surgery.

'First Nose Surgery in Your 20s'

Unlike Western noses, East Asian noses often tend to be low and flat.

For that reason, while nose surgery in the West developed techniques to reduce a large nose and remove a hump, nose surgery in East Asia developed techniques to raise a low bridge and create a more defined tip.

Septal cartilage is partially harvested from inside the nose during nose surgery, and an appropriate amount must be left behind so that the nasal structure does not collapse.

Because the nose is usually small in East Asians, the septal cartilage is often small as well, so ear cartilage is often additionally harvested and used as a material for tip surgery.

To raise the bridge, a silicone implant is used, and an implant height that is appropriate among options ranging from about 2 mm to 6–7 mm is selected.

At the tip, the harvested septal cartilage is used to perform a septal extension graft.

A septal extension graft is a very important procedure in tip surgery, and it can increase the height of the tip or extend its length.

Depending on the person, it can be fixed in an appropriate position and direction to shape the tip.

Ear cartilage is soft in texture and slightly curved, so it is mainly used as an onlay graft method, where cartilage is placed on the tip to increase height.

This procedure supplements the areas that could not be raised with septal cartilage, and has the advantage of keeping the tip from becoming too rigid.

Finally, the surgery is completed by performing a dermis graft on the nasal tip so that the tip skin does not thin over the long term.

A complication in which the skin of the nasal tip becomes thinner is fairly common, and if it can be prevented, it should be prevented in advance to avoid revision surgery in the long term.

'Nostril Reduction (Alar Reduction)'

If the nostrils are wide together with first nose surgery, the nostrils can also be further reduced to create a more refined appearance.
